摘要:
目的 探讨ST段抬高型心肌梗死(STEMI)患者术前中性粒细胞与淋巴细胞比值(NLR)与冠脉病变程度的关系。方法 选取2019年5月~2020年5月在我院心内科住院并行PCI术的STEMI患者210例作为研究对象,根据Gensini积分法对冠脉狭窄程度评分,分为单支病变组、双支病变组、三支病变组,并收集STEMI患者术前NLR,分出高NLR组(>3.23)和低NLR组(<3.23),分析术前NLR与冠脉病变程度的相关性。结果 高NLR组在白细胞计数、中性粒细胞计数、吸烟史高于低NLR组(P<0.05);三支病变组较单支病变组、双支病变组NLR高,差异有统计学意义(P<0.05);多因素Logistic分析显示,吸烟(OR:2.298,95%CI:1.018~5.187,P=0.045)、中性粒细胞计数(OR:4.173,95%CI:1.809~8.973,P=0.001)、NLR(OR:3.930,95%CI:1.736 ~8.898,P=0.001)均是冠心病冠脉病变程度独立危险因素。结论 STEMI患者术前NLR在冠状动脉狭窄程度上有一定的预测价值,较高水平NLR提示冠脉狭窄程度越重。
Abstract:
Objective To observe the relationship between pre-operation neutrophil to lymphocyte ratio(NLR) and coronary artery stenosis degree in patients with ST-segment elevation myocardial infarction(STEMI).Methods A total of 210 STEMI patients who were hospitalized with PCI in the cardiology department of our hospital from May 2019 to May 2020 were selected as the study objects. According to the Gensini score method, the coronary artery stenosis was scored and divided into single vessel disease group and double vessel disease group Three-vessel disease group, and preoperative NLR of STEMI patients were collected, divided into high NLR group (>3.23) and low NLR group (<3.23), and analyzed the correlation between preoperative NLR and the degree of coronary artery disease.Results The high NLR group had higher white blood cell count, neutrophil count, and smoking history than the low NLR group (P<0.05); the three-vessel disease group had higher NLR than the single-vessel disease group and the double-vessel disease group,the difference was statistically significant (P<0.05); Multivariate Logistic analysis showed that smoking (OR:2.298, 95%CI:1.018~5.187,P=0.045), neutrophil count (OR:4.173, 95%CI: 1.809~8.973,P=0.001), NLR (OR:3.930, 95%CI: 1.736 ~ 8.898,P=0.001) are independent risk factors for the degree of coronary artery disease.Conclusion Preoperative NLR in STEMI patients has a certain predictive value in the degree of coronary artery stenosis. A higher level of NLR indicates more severe coronary stenosis.
