卒中后抑郁(PSD)是脑卒中后一种常见的并发症,具有较高的发病率,但PSD的发病机制仍未明确。随着神经影像技术的发展,近年来许多研究表明脑情感网络的损伤与PSD的发生发展密切相关,然而脑情感网络损伤的病理机制仍有待于进一步阐明,是目前研究的热点之一。本文主要综述PSD与脑情感网络受损的关系,从卒中后脑灌注异常及脑组织缺血后继发性损害角度分析脑情感网络受损可能的病理机制,旨在为PSD的早期防控提供可干预的临床思路。
Abstract:
Post-stroke depression (PSD) is a common complication after stroke, with a high incidence, but the pathogenesis of PSD is still unclear. With the development of neuroimaging technology, many studies in recent years have shown that brain-emotional network damage is closely related to the occurrence and development of PSD. However, the pathological mechanism of brain-emotional network damage still needs to be further clarified, which is one of the current research hotspots.This article mainly reviews the relationship between PSD and brain affective network damage, and analyzes the possible pathological mechanism of brain affective network damage from the perspective of abnormal cerebral perfusion after stroke and secondary damage to brain tissue after ischemia, aiming to provide interventions for the early prevention and control of PSD clinical thinking.
