摘要:
目的 观察青光眼患者局部应用降眼压药物后视盘血流密度的变化。方法 选择2019年2月~9月我院门诊收治的原发性青光眼患者32例(48眼)作为研究对象,局部应用2%盐酸卡替洛尔滴眼液、布林佐胺滴眼液和酒石酸溴莫尼定滴眼液治疗,比较患者治疗前、用药1周、1个月时眼压、视网膜神经纤维层厚度(RNFL)、视网膜神经节细胞复合体(GCC)厚度、C/D 值、视盘整体血流密度及盘周血流密度,分析影响视盘血流密度的相关因素。结果 用药1周、1个月,患者眼压分别为(15.98±2.70)mmHg和(16.47±4.15)mmHg,均低于用药前的(31.65±5.35)mmHg,差异有统计学意义(P<0.05);用药前,视盘整体及盘周血流密度分别为(49.64±4.87)%和(37.72±3.25)%,用药1周视盘整体及盘周血流密度为(52.14±5.30)%和(39.91±3.33)%,1个月后为(52.13±5.13)%和(40.03±3.37)%,均较用药前增加,差异有统计学意义(P<0.05);用药1周、1个月C/D 值、RNFL厚度、GCC厚度与用药前比较,差异无统计学意义(P>0.05);视盘整体血流密度和盘周血流密度的增加量与眼压下降量、C/D值、RNFL厚度、GCC厚度的变化量无相关性(P>0.05)。结论 青光眼患者局部应用降眼压药物后眼压改善效果较好,视盘整体血流密度及盘周血流密度均增加,视盘血流状态得到改善。
Abstract:
Objective To observe the changes of optic disc blood flow density after local application of intraocular pressure lowering drugs in glaucoma patients.Methods 32 cases (48 eyes) of primary glaucoma patients admitted to our hospital from February to September 2019 were selected as the research objects.Topical application of 2% carteolol hydrochloride eye drops, brinzolamide eye drops and brimonidine tartrate eye drops for treatment,compare patients’ intraocular pressure, retinal nerve fiber layer thickness (RNFL), retinal ganglion cell complex (GCC) thickness, C/D value,the overall optic disc blood flow density and peridiscal blood flow density were analyzed, and the related factors affecting optic disc blood flow density were analyzed.Results The intraocular pressures of the patients were (15.98 ± 2.70) mmHg and (16.47 ± 4.15) mmHg respectively after 1 week and 1 month of medication, which were lower than the (31.65 ± 5.35) mmHg before medication,the difference was statistically significant (P<0.05);Before medication, the blood flow density of the entire optic disc and peridiscal was (49.64±4.87)% and (37.72±3.25)%, respectively.The blood flow density of the entire optic disc and peridiscal was (52.14±5.30)% and (39.91±3.33)% during the first week of medication.One month later, it was (52.13±5.13)% and (40.03±3.37)%, both of which increased compared with before medication,the difference was statistically significant (P<0.05);The C/D value, RNFL thickness and GCC thickness of 1 week and 1 month after medication were compared with those before medication,there was no significant difference (P>0.05);There was no correlation between the increase in the overall optic disc blood flow density and the peridiscal blood flow density and the decrease in intraocular pressure, C/D value, RNFL thickness, and GCC thickness (P>0.05).Conclusion After local application of intraocular pressure lowering drugs in glaucoma patients, the effect of improving intraocular pressure is better. The blood flow density of the whole optic disc and the blood flow density of the disc are increased, and the blood flow status of the optic disc is improved.
