摘要:
目的 探讨多排螺旋CT(MSCT)冠脉造影(CT-CA)和心脏成像在冠心病中的临床应用价值。方法 选择2015年4月~2018年3月我院收治的冠心病患者52例,采用心电门控MSCT心脏扫描后行冠状动脉造影和心脏重建,测量冠脉狭窄度和左心室功能指标,分别与DSA冠脉造影(DSA-CA)和超声心动图比较。结果 本组患者780个冠状动脉节段中,DSA-CA检出狭窄节段146个,CT-CA检出144个,诊断敏感度为93.15%,特异度为98.67%,阳性预测值为94.44%,阴性预测值为98.34%,一致性Kappa检验k=0.927;MSCT心脏成像与超声心电图检测的左心室功能指标比较,差异无统计学意义(P>0.10)。结论 MSCT可一次性完成冠心病的冠脉狭窄测量和左心室功能评价,可获得与DSA冠脉造影、超声心电图相同的效果,具有良好应用价值。
Abstract:
Objective To explore the clinical application value of multi-slice spiral CT (MSCT) coronary angiography (CT-CA) and cardiac imaging in coronary heart disease.Methods 52 patients with coronary heart disease admitted to our hospital from April 2015 to March 2018 were selected for coronary angiography and cardiac reconstruction after ECG-gated MSCT cardiac scanning.Measure coronary stenosis and left ventricular function indexes, and compare with DSA coronary angiography (DSA-CA) and echocardiography.Results Among the 780 coronary artery segments in this group of patients, 146 stenotic segments were detected by DSA-CA and 144 were detected by CT-CA. The diagnostic sensitivity was 93.15%.The specificity was 98.67%, the positive predictive value was 94.44%, the negative predictive value was 98.34%, the consistency Kappa test k=0.927;There was no statistically significant difference in left ventricular function between MSCT cardiac imaging and echocardiography (P>0.10).Conclusion MSCT coronary angiography and cardiac imaging can complete the coronary stenosis measurement and left ventricular function evaluation of coronary heart disease at one time. It can obtain the same effect as DSA coronary angiography and echocardiography, and has good application value.
