摘要:
目的 探讨胃息肉发生的危险因素,为预防胃息肉的发生提供科学依据。方法 选取2015年1月1日~2020年1月1日就诊于石河子大学第一附属医院经胃镜活检诊断为胃息肉的240例患者作为胃息肉组,另选取同期胃镜检查未被诊断为胃息肉的240名体检者作为非胃息肉组,收集研究对象的临床资料,分析胃息肉的危险因素。结果 两组性别、BMI、吸烟、饮酒、饮水温度、吃水果的量、吃蔬菜的量、体育锻炼及焦虑程度、PPI药物使用、Hp感染、胃息肉家族史、高脂蛋白血症、胆囊炎比较,差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示,性别、BMI、吸烟、饮水温度、焦虑程度、PPI使用情况、Hp感染结果、高脂蛋白血症、胃息肉家族史、吃水果的量、吃蔬菜的量、体育锻炼是胃息肉的影响因素(P<0.05);胃息肉病理特征及分布部位与性别、BMI、吸烟、饮水温度、吃水果的量、吃蔬菜的量、体育锻炼、焦虑程度、高脂蛋白血症、PPI使用情况、Hp感染结果、胃息肉家族史无关,差异无统计学意义(P>0.05);胃息肉数量与性别、BMI、吸烟、饮水温度、焦虑程度、高脂蛋白血症、PPI使用情况、Hp感染、胃息肉家族史、吃水果的量、吃蔬菜的量、体育锻炼无关,差异无统计学意义(P>0.05),与PPI使用有关,差异有统计学意义(P<0.05)。结论 建议女性、BMI≥24 kg/m2、长期吸烟、饮温热水及烫水、长期服用PPI、HP感染阳性、重度焦虑、高脂蛋白血症、有胃息肉家族史者完善胃镜检查,发现胃息肉及时处理并治疗,保持体育锻炼、多进食蔬菜水果等良好生活习惯可以降低胃息肉的发生。
Abstract:
Objective To explore the risk factors of gastric polyps and provide scientific basis for preventing gastric polyps.Methods A total of 240 patients who were diagnosed with gastric polyps by gastroscopic biopsy from January 1, 2015 to January 1, 2020 in the First Affiliated Hospital of Shihezi University were selected as the gastric polyp group.In addition, 240 physical examiners who were not diagnosed with gastric polyps during gastroscopy during the same period were selected as the non-gastric polyps group, and clinical data of the study subjects were collected to analyze the risk factors of gastric polyps.Results Two groups of gender, BMI, smoking, drinking, drinking water temperature, the amount of fruits eaten, the amount of vegetables eaten, physical exercise and anxiety, PPI drug use, Hp infection, family history of gastric polyps, hyperlipoproteinemia, cholecystitis in comparison, the difference was statistically significant (P<0.05);The results of multivariate Logistic regression analysis showed that gender, BMI, smoking, drinking water temperature, anxiety level, PPI usage, Hp infection results, hyperlipoproteinemia, family history of gastric polyps, the amount of fruits eaten, the amount of vegetables eaten, and physical exercise were the influencing factors of gastric polyps (P<0.05);Pathological characteristics and distribution of gastric polyps and gender, BMI, smoking, drinking water temperature, the amount of fruits eaten, the amount of vegetables eaten, physical exercise, anxiety, hyperlipoproteinemia, PPI usage, Hp infection results, gastric polyp family history was unrelated,the difference was not statistically significant (P>0.05);The number of gastric polyps had nothing to do with gender, BMI, smoking, drinking water temperature, anxiety level, hyperlipoproteinemia, PPI usage, Hp infection, family history of gastric polyps, the amount of fruits eaten, the amount of vegetables eaten, or physical exercise,the difference was not statistically significant (P>0.05),it was related to the use of PPI,the difference was statistically significant (P<0.05).Conclusion It is recommended that women, BMI ≥ 24 kg/m2, long-term smoking, drinking warm hot water and hot water, long-term use of PPI, Hp infection positive, severe anxiety, hyperlipoproteinemia, and family history of gastric polyps are recommended to complete gastroscopy and find gastric polyps in time Treatment and treatment, maintaining good habits such as physical exercise and eating more fruits and vegetables can reduce the occurrence of gastric polyps.
