摘要:
目的 研究居家维护模式在PICC带管肿瘤出院患者中的安全性和有效性。方法 选取2016年10月-2020年9月我院进行PICC置管的144例出院肿瘤患者为研究对象，采用随机数字表法分为对照组和观察组，各72例。对照组实施返院PICC专科门诊维护模式，观察组实施居家维护模式。比较两组PICC置管时间、维护依从率、自我管理能力评分、护理满意度、维护费用和时间以及并发症发生率。结果 观察组平均PICC置管时间与对照组比较，差异无统计学意义（P＞0.05）；观察组维护依从率为93.05%，高于对照组的83.33%（P＜0.05）；观察组自我管理能力评分高于对照组（P＜0.05）；观察组护理满意度为94.44%，高于对照组的88.89%（P＜0.05）；观察组单次平均维护费用、时间均小于对照组（P＜0.05）；观察组过敏性皮炎、穿刺口感染、导管脱出、弹力性水泡、导管堵塞、深静脉血栓发生率与对照组比较，差异无统计学意义（P＞0.05）。结论 居家维护模式在PICC带管肿瘤出院患者可提高患者维护依从性、自我护理能力以及护理满意度，且不会增加并发症，具有良好的应用安全性和有效性。
Abstract:
Objective To study the safety and effectiveness of home maintenance mode in PICC patients with tube-in-tube tumors discharged from the hospital.Methods A total of 144 discharged tumor patients who underwent PICC catheterization in our hospital from October 2016 to September 2020 were selected as the research objects. They were divided into control group and observation group by random number table method, with 72 cases in each group.The control group implemented the return-to-hospital PICC specialist outpatient maintenance model, and the observation group implemented the home maintenance model.The PICC intubation time, maintenance compliance rate, self-management ability score, nursing satisfaction, maintenance cost and time, and complication rate were compared between the two groups.Results There was no statistically significant difference in the average PICC placement time between the observation group and the control group (P＞0.05);The maintenance compliance rate of the observation group was 93.05%, which was higher than 83.33% of the control group(P＜0.05);The self-management ability score of the observation group was higher than that of the control group(P＜0.05);The nursing satisfaction rate of the observation group was 94.44%, which was higher than 88.89% of the control group(P＜0.05);The average single maintenance cost and time of the observation group were less than those of the control group(P＜0.05);The incidence of allergic dermatitis, puncture site infection, catheter prolapse, elastic blisters, catheter blockage and deep vein thrombosis between the observation group and the control group was not statistically different (P＞0.05).Conclusion The home maintenance mode can improve patient maintenance compliance, self-care ability and nursing satisfaction in PICC patients with tumor discharged from the hospital, without increasing complications, and has good application safety and effectiveness.
