摘要:
目的 探究血清总胆汁酸(TBA)、胆碱酯酶(CHE)和前白蛋白(PA)检测在肝病诊断中的应用价值。方法 选取2019年6月-2021年6月经天津市第二人民医院确诊的101例肝病患者为研究对象,其中包括慢性病毒性肝炎32例,急性病毒性肝炎24例,肝炎型肝硬化25例,原发性肝癌20例;另选取35例同期体检健康者为对照组,比较其TBA、CHE、PA水平及阳性检出率,分析以上指标与肝功能Child-Pugh分级的相关性进行分析。结果 不同肝病类型患者的血清TBA、CHE、PA水平均高于对照组(P<0.05),其TBA由高至低依次为急性病毒性肝炎>原发性肝癌>肝炎型肝硬化>慢性病毒性肝炎>对照组,CHE、PA由高至低依次为对照组>慢性病毒性肝炎>急性病毒性肝炎>肝炎型肝硬化>原发性肝癌。不同肝病类型的TBA、CHE、PA阳性检出率均高于对照组(P<0.05),其阳性检出率由高到低依次为原发性肝癌>肝炎型肝硬化>急性病毒性肝炎>慢性病毒性肝炎>对照组;不同Child-Pugh分级间TBA、CHE、PA水平比较,差异有统计学意义(P<0.05);Pearson相关性分析显示,TBA与Child-Pugh分级呈正相关(r=0.634,P<0.05),CHE、PA与Child-Pugh分级呈负相关(r=-0.283、-0.352,P<0.05)。结论 TBA、CHE、PA在肝病诊断中具有确切的指导作用,可有效反映肝脏代谢功能及肝细胞的受损情况,在不同病程阶段中均具有较高的阳性检出率,且与肝功能Child-Pugh分级存在明显相关性。
Abstract:
Objective To explore the application value of serum total bile acid (TBA), cholinesterase (CHE) and prealbumin (PA) detection in the diagnosis of liver diseases.Methods A total of 101 patients with liver diseases diagnosed in Tianjin Second People ’s Hospital from June 2019 to June 2021 were selected as study subjects, including 32 cases of chronic viral hepatitis, 24 cases of acute viral hepatitis, 25 cases of hepatitis cirrhosis and 20 cases of primary liver cancer. Another 35 healthy subjects were selected as the control group. The levels of TBA, CHE and PA and the positive detection rate were compared, and the correlation between the above indexes and the Child-Pugh classification of liver function was analyzed.Results The levels of serum TBA, CHE and PA in patients with different types of liver disease were higher than those in control group (P<0.05), and TBA from high to low was acute viral hepatitis>primary liver cancer>hepatitis cirrhosis>chronic viral hepatitis>control group, the CHE and PA from high to low was control group>chronic viral hepatitis>acute viral hepatitis>hepatitis cirrhosis>primary liver cancer. The positive detection rates of TBA, CHE and PA in different liver diseases were higher than those in the control group (P<0.05), and the positive detection rates from high to low were primary liver cancer>hepatitis cirrhosis>acute viral hepatitis>chronic viral hepatitis>control group; the differences in TBA, CHE and PA levels between different Child-Pugh grades were statistically significant (P<0.05). Pearson correlation analysis showed that TBA was positively correlated with Child-Pugh classification (r=0.634, P<0.05), CHE and PA were negatively correlated with Child-Pugh classification (r=-0.283, -0.352, P<0.05).Conclusion TBA, CHE, and PA have a definite guiding role in the diagnosis of liver diseases, which can effectively reflect the metabolic function of liver and the damage of liver cells. They have high positive detection rates in different stages of disease, and are significantly correlated with Child-Pugh classification of liver function.
