摘要:
目的 探讨鼻内镜手术治疗慢性鼻窦炎伴鼻息肉的临床疗效及对生活质量的影响。方法 选取2017年1月-2020年12月在我院治疗的104例慢性鼻窦炎伴鼻息肉患者为研究对象，采用随机数字表法分为对照组和观察组，各52例。对照组给予常规术式治疗，观察组给予鼻内镜手术治疗。比较两组临床疗效、手术相关指标、生活质量、临床症状评分、并发症发生率。结果 观察组治疗总有效率为94.23%，高于对照组的82.69%，差异有统计学意义（P＜0.05）；观察组手术时间、术中出血量、术后鼻腔恢复通气时间均小于对照组，差异有统计学意义（P＜0.05）；观察组生活质量生理功能、躯体疼痛、社会功能、情感功能各维度评分均高于对照组，差异有统计学意义（P＜0.05）；观察组临床鼻塞、鼻涕、水肿、头痛各症状评分均低于对照组，差异有统计学意义（P＜0.05）；观察组并发症发生率为5.76%，低于对照组的15.38%，差异有统计学意义（P＜0.05）。结论 鼻内镜手术治疗慢性鼻窦炎伴鼻息肉的临床疗效确切，手术时间短，术中出血量少，术后恢复快，可显著减轻术后头痛、水肿、鼻塞等症状，改善患者生活质量，且并发症发生率少，具有良好的治疗安全性。
Abstract:
Objective To evaluate the clinical effect of endoscopic sinus surgery on chronic sinusitis with nasal polyps and its effect on quality of life.Methods A total of 104 patients with chronic sinusitis and nasal polyps treated in our hospital from January 2017 to December 2020 were selected as the research objects and randomly divided into the control group and the observation group, with 52 cases in each group. The control group was given routine surgical treatment, and the observation group was given nasal endoscopic surgery. The clinical efficacy, surgical related indexes, quality of life, clinical symptom score and incidence of complications were compared between the two groups.Results The total effective rate of the observation group was 94.23%, which was higher than 82.69% of the control group, and the difference was statistically significant (P<0.05). The operation time, intraoperative blood loss and postoperative nasal ventilation recovery time in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). The scores of physiological function, physical pain, social function and emotional function of quality of life in the observation group were higher than those in the control group, and the difference was statistically significant (P<0.05). The scores of clinical symptoms of nasal obstruction, nose, edema and headache in the observation group were lower than those in the control group, and the difference was statistically significant (P<0.05). The incidence of complications in the observation group was 5.76%, which was lower than 15.38% in the control group, and the difference was statistically significant (P<0.05).Conclusion Endoscopic sinus surgery can effectively treat chronic sinusitis with nasal polyps, which has the advantages of short operation time, less intraoperative blood loss, and rapid postoperative recovery. It can also significantly reduce postoperative headache, edema, nasal congestion and other symptoms, improve the quality of life of patients, and reduce the incidence of complications, has good therapeutic safety.
