摘要:
目的 观察曼月乐与地屈孕酮治疗围绝经期异常子宫出血的疗效及对性激素水平的影响。方法 选取2020年3月-2021年3月在我院诊治的108例围绝经期异常子宫出血患者作为研究对象，采用随机数字表法分为对照组和观察组，各54例。对照组采用地屈孕酮治疗，观察组采用曼月乐治疗，比较两组治疗总有效率、子宫内膜厚度、月经量、血红蛋白、性激素水平及不良反应发生情况。结果 观察组总有效率为94.44%，高于对照组的85.19%，差异有统计学意义（P＜0.05）；两组子宫内膜厚度、月经量均小于治疗前，血红蛋白高于治疗前，且观察组子宫内膜厚度、月经量小于对照组，血红蛋白大于对照组，差异有统计学意义（P＜0.05）；两组雌二醇（E2）、卵泡刺激素（FSH）及黄体生成素（LH）水平均低于治疗前，观察组低于对照组，差异有统计学意义（P＜0.05）；观察组不良反应发生率为5.56%，低于对照组的12.96%，差异有统计学意义（P＜0.05）。结论 曼月乐与地屈孕酮治疗围绝经期异常子宫出血均具有一定疗效，但与地屈孕酮比较，曼月乐可提高治疗总有效率，减少月经量和子宫内膜厚度，改善性激素水平，且不良反应发生率低，安全性较高，值得临床应用。
Abstract:
Objective To observe the curative effect of Mirena and Dydrogesterone in the treatment of perimenopausal abnormal uterine bleeding and its effect on sex hormone levels.Methods A total of 108 patients with perimenopausal abnormal uterine bleeding who were treated in our hospital from March 2020 to March 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 54 cases in each group. The control group was treated with Dydrogesterone, and the observation group was treated with Mirena. The total effective rate, endometrial thickness, menstrual volume, hemoglobin, sex hormone levels and adverse reactions of the two groups were compared.Results The total effective rate of the observation group was 94.44%, which was higher than 85.19% of the control group, and the difference was statistically significant (P<0.05). The endometrial thickness and menstrual volume of the two groups were less than those before treatment, and the hemoglobin was higher than that before treatment, while the endometrial thickness and menstrual volume of the observation group were less than those of the control group, and the hemoglobin was greater than that of the control group, the difference was statistically significant (P<0.05). The levels of estradiol (E2), follicle stimulating hormone (FSH) and luteinizing hormone (LH) in the two groups were lower than those before treatment, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). The incidence of adverse reactions in the observation group was 5.56%, which was lower than 12.96% in the control group, and the difference was statistically significant (P<0.05).Conclusion Mirena and Dydrogesterone have certain curative effects in the treatment of perimenopausal abnormal uterine bleeding. However, compared with Dydrogesterone, Mirena can improve the total effective rate, reduce menstrual volume and endometrial thickness, and improve the level of sexual hormones. Moreover, it has low incidence of adverse reactions and high safety, which is worthy of clinical application.
