摘要:
目的 研究超声引导下颈神经根阻滞治疗慢性颈源性疼痛的临床效果。方法 选取2021年5月-2022年5月在我院诊治的72例慢性颈源性疼痛患者为研究对象,采用随机数字表法分为对照组和观察组,各组36例。对照组采用盲穿下颈部侧入路颈神经根阻滞治疗,观察组采用超声引导下颈神经根阻滞治疗,比较两组临床疗效、不同时间疼痛(VAS)评分、炎症因子[白细胞(WBC)、C-反应蛋白(CRP)]水平、颈椎功能障碍指数(NDI)、颈椎活动度(ROM)评分、疼痛次数及疼痛持续时间。结果 观察组治疗总有效率为94.44%,高于对照组的80.56%(P<0.05);治疗1、6、24 h,两组疼痛评分均低于治疗前,治疗1 h两组疼痛评分比较,差异无统计学意义(P>0.05),治疗6、24 h观察组疼痛评分低于对照组(P<0.05);两组WBC、CRP均低于治疗前,且观察组低于对照组(P<0.05);两组NDI、ROM评分均低于治疗前,且观察组低于对照组(P<0.05);观察组疼痛次数、疼痛持续时间均少于对照组(P<0.05)。结论 超声引导下颈神经根阻滞治疗慢性颈源性疼痛临床效果确切,可提高治疗效果,降低患者治疗后6~24 h疼痛评分,改善炎症反应,减小疼痛次数,缩短疼痛持续时间,改善患者颈椎功能,促进颈椎活动度恢复,值得临床应用。
Abstract:
Objective To study the clinical effect of ultrasound-guided cervical nerve root block in the treatment of chronic cervical pain.Methods A total of 72 patients with chronic cervical pain diagnosed and treated in our hospital from May 2021 to May 2022 were selected as the research objects. They were divided into control group and observation group by random number table method, with 36 cases in each group. The control group was treated with cervical nerve root block by blind puncture of cervical lateral approach, and the observation group was treated with ultrasound-guided cervical nerve root block. The clinical efficacy, pain (VAS) score at different times, inflammatory factors [white blood cell (WBC), C-reactive protein (CRP)] levels, cervical dysfunction index (NDI), cervical range of motion (ROM) score, pain frequency and pain duration were compared between the two groups.Results The total effective rate of the observation group was 94.44%, which was higher than 80.56% of the control group (P<0.05). At 1, 6 and 24 h after treatment, the pain scores of the two groups were lower than those before treatment, while there was no significant difference in pain scores between the two groups at 1 h after treatment (P>0.05). The pain scores of the observation group were lower than those of the control group at 6 and 24 h after treatment (P<0.05). The WBC and CRP of the two groups were lower than those before treatment, and those of the observation group were lower than the control group (P<0.05). The NDI and ROM scores of the two groups were lower than those before treatment, and those of the observation group were lower than the control group (P<0.05). The number and duration of pain in the observation group were less than those in the control group (P<0.05).Conclusion Ultrasound-guided cervical nerve root block has a definite clinical effect in the treatment of chronic cervicogenic pain, which can improve the therapeutic effect, reduce the pain score of patients at 6-24 h after treatment, improve the inflammatory response, reduce the number of pain, shorten the duration of pain, improve the cervical function of patients, and promote the recovery of cervical mobility. It is worthy of clinical application.
