摘要:
目的 观察关节镜辅助下治疗SchatzkerⅠ~Ⅳ型胫骨平台骨折的疗效。方法 选取2020年10月-2021年10月在我院诊治的74例SchatzkerⅠ~Ⅳ型胫骨平台骨折患者为研究对象,采用随机数字表法分为对照组和观察组,各37例。对照组采用传统切开复位内固定治疗,观察组采用关节镜辅助下微创经皮钢板内固定治疗,比较两组临床手术指标、骨折恢复优良率、骨折愈合时间、膝关节功能(HSS)评分、胫骨平台功能指标及并发症。结果 观察组术中出血量、术后下床活动时间、术后引流量、手术切口均小于对照组(P<0.05);观察组骨折恢复优良率为94.59%,高于对照组的81.08%(P<0.05);观察组骨折愈合时间短于对照组,HSS评分高于对照组(P<0.05);观察组膝关节间隙宽度小于对照组(P<0.05),两组PA、TPA、FTA比较,差异无统计学意义(P>0.05);观察组并发症发生率(8.11%)低于对照组(18.92%)(P<0.05)。结论 关节镜辅助下治疗SchatzkerⅠ~Ⅳ型胫骨平台骨折效果确切,可减少术中出血量和术后引流量,缩短术后下床活动时间,提升膝关节功能评分,改善胫骨平台各指标,提高骨折恢复优良率,降低并发症发生率,值得临床应用。
Abstract:
Objective To observe the efficacy of arthroscopic assisted treatment of SchatzkerⅠ-Ⅳ tibial plateau fractures.Methods A total of 74 patients with Schatzker I-IV tibial plateau fractures treated in our hospital from October 2020 to October 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 37 cases in each group. The control group was treated with traditional open reduction and internal fixation, and the observation group was treated with arthroscopic assisted minimally invasive percutaneous plate internal fixation. The clinical operation indexes, excellent and good rate of fracture recovery, fracture healing time, knee joint function (HSS) score, tibial plateau function index and complications were compared between the two groups.Results The intraoperative blood loss, postoperative ambulation time, postoperative drainage volume and surgical incision in the observation group were less than those in the control group (P<0.05). The excellent and good rate of fracture recovery in the observation group was 94.59%, which was higher than 81.08% in the control group (P<0.05). The fracture healing time of the observation group was shorter than that of the control group, and the HSS score was higher than that of the control group (P<0.05). The width of knee joint space in the observation group was smaller than that in the control group (P<0.05). There was no significant difference in PA, TPA and FTA between the two groups (P>0.05). The incidence of complications in the observation group (8.11%) was lower than that in the control group (18.92%) (P<0.05).Conclusion Arthroscopic assisted treatment of Schatzker Ⅰ-Ⅳ tibial plateau fractures is effective, which can reduce intraoperative blood loss and postoperative drainage, shorten postoperative ambulation time, improve knee function score, improve tibial plateau indicators, improve fracture recovery rate, and reduce the incidence of complications. It is worthy of clinical application.
