摘要:
目的 观察奥曲肽联合泮托拉唑治疗老年消化性溃疡（PU）合并上消化道出血（UGH）的疗效及对凝血功能和炎症因子的影响。方法 选取2019年1月-2021年1月万安县人民医院收治的72例老年PU合并UGH患者为研究对象，经随机数字表法分为对照组和观察组，每组36例。对照组给予泮托拉唑治疗，观察组给予奥曲肽联合泮托拉唑治疗，比较两组止血疗效、止血时间、胃液pH值、血红蛋白、凝血功能[凝血酶原时间（PT）、凝血酶时间（TT）、纤维蛋白原（Fib）]、炎症因子[超敏-C反应蛋白（hs-CRP）、肿瘤坏死因子-α（TNF-α）、白介素-6（IL-6）]。结果 观察组止血有效率为91.67%，高于对照组的80.56%（P＜0.05）；观察组治疗后胃液pH值、血红蛋白高于对照组，止血时间短于对照组（P＜0.05）；观察组PT、TT、Fib低于对照组（P＜0.05）；观察组hs-CRP、TNF-α、IL-6水平低于对照组（P＜0.05）。结论 奥曲肽联合泮托拉唑对老年PU合并UGH具有确切止血效果，有助于缩短止血时间，调节患者胃内pH值及血红蛋白水平，改善凝血功能，同时下调炎症反应。
Abstract:
Objective To observe the efficacy of octreotide combined with pantoprazole in the treatment of elderly peptic ulcer (PU) with upper gastrointestinal hemorrhage (UGH) and its effect on coagulation function and inflammatory factors.Methods A total of 72 elderly patients with PU and UGH admitted to Wan’an County People ’s Hospital from January 2019 to January 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 36 cases in each group. The control group was treated with pantoprazole, and the observation group was treated with octreotide combined with pantoprazole. The hemostatic efficacy, hemostatic time, gastric juice pH, hemoglobin, coagulation function [prothrombin time (PT), thrombin time (TT), fibrinogen (Fib)], inflammatory factors [high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6)] were compared between the two groups.Results The effective rate of hemostasis in the observation group was 91.67%, which was higher than 80.56% in the control group (P<0.05). After treatment, the pH value of gastric juice and hemoglobin in the observation group were higher than those in the control group, and the hemostasis time was shorter than that in the control group (P<0.05). PT, TT and Fib in the observation group were lower than those in the control group (P<0.05). The levels of hs-CRP, TNF-α and IL-6 in the observation group were lower than those in the control group (P<0.05).Conclusion Octreotide combined with pantoprazole has a definite hemostatic effect on elderly PU combined with UGH, which is helpful to shorten the hemostatic time, regulate the pH value and hemoglobin level in the stomach, improve the coagulation function, and reduce the inflammatory response.
