摘要:
目的 观察彩色多普勒超声评估透析患者动静脉内瘘血栓及狭窄的临床价值。方法 选取2020年1月-2021年8月在井冈山大学附属医院血液透析治疗出现血流量不足表现的62例患者为研究对象，均以血管造影为金标准。比较血管造影与彩色多普勒超声诊断透析患者动静脉内瘘血栓、狭窄以及血栓+狭窄诊断准确率、彩色多普勒超声诊断血栓和狭窄的血流动力学特征、诊断效能（灵敏度、特异度）、与血管造影诊断一致性以及超声表现特点。结果 彩色多普勒超声动静脉内瘘血栓诊断准确率与血管造影相同，狭窄诊断准确性、血栓+狭窄诊断准确率均低于血管造影（P＜0.05）；彩色多普勒超声诊断，动静脉内瘘血栓血流量、收缩期峰值流速（PSV）均低于狭窄患者，阻力指数（RI）高于狭窄患者（P＜0.05）；彩超血流量评估内瘘血栓及狭窄的ROC曲线下面积为0.836，当血流量取＜350.45 ml/min时，灵敏度为87.69%，特异度为83.33%；彩色多普勒超声诊断动静脉内瘘血栓准确率与血管造影诊断结果具有高度一致性，且一致性高于狭窄+血栓、狭窄单独诊断准确率（P＜0.05）。结论 彩色多普勒超声评估血液透析患者动静脉内瘘血栓及狭窄具有较高的临床价值，且在狭窄和血栓超声征象、血流动力学方面存在差异，可促进血栓和狭窄鉴别诊断，实现较理想的诊断效能，为临床预防血栓和狭窄以及选择合适的治疗方案提供可靠参考依据。
Abstract:
Objective To observe the clinical value of color Doppler ultrasound in evaluating arteriovenous fistula thrombosis and stenosis in dialysis patients.Methods From January 2020 to August 2021,62 patients with insufficient blood flow in hemodialysis treatment in the Affiliated Hospital of Jinggangshan University were selected as the research objects, and angiography was the gold standard. The diagnostic accuracy of angiography and color Doppler ultrasound in the diagnosis of thrombosis, stenosis, thrombosis and stenosis of arteriovenous fistula in dialysis patients, the hemodynamic characteristics, diagnostic efficacy (sensitivity, specificity), consistency with angiography and ultrasound features of thrombosis and stenosis diagnosed by color Doppler ultrasound were compared.Results The diagnostic accuracy of color Doppler ultrasound for arteriovenous fistula thrombosis was the same as that of angiography, and the diagnostic accuracy of stenosis and thrombosis+stenosis was lower than that of angiography(P<0.05). The thrombus blood flow and peak systolic velocity (PSV) of arteriovenous fistula diagnosed by color Doppler ultrasound were lower than those of stenosis patients, and the resistance index (RI) was higher than that of stenosis patients (P<0.05). The area under the ROC curve was 0.836. When the blood flow was <350.45 ml/min, the sensitivity was 87.69% and the specificity was 83.33%. The accuracy of color Doppler ultrasound in the diagnosis of arteriovenous fistula thrombosis was highly consistent with that of angiography, and the consistency was higher than that of stenosis + thrombosis and stenosis alone (P<0.05).Conclusion Color Doppler ultrasound has high clinical value in evaluating arteriovenous fistula thrombosis and stenosis in hemodialysis patients, and there are differences in ultrasound signs and hemodynamics of stenosis and thrombosis, which can promote the differential diagnosis of thrombosis and stenosis, achieve better diagnostic efficacy, and provide a reliable reference for clinical prevention of thrombosis and stenosis and selection of appropriate treatment options.
