摘要:
目的 探讨腹腔镜胆总管切开取石术对胆总管结石伴胆囊结石患者胃肠道功能的影响。方法 选取2020年1月-2021年1月我院诊治的54例胆总管结石伴胆囊结石患者为研究对象，采用随机数字表法分为对照组和观察组，各27例。对照组采用传统开腹胆囊切除与胆总管取石手术治疗，观察组采用腹腔镜胆总管切开取石术治疗，比较两组临床手术指标、胃功能恢复指标、结石清除率、不同时间段疼痛（VAS）评分、并发症发生率、复发率。结果 观察组手术时间、下床活动时间短于对照组、术中出血量少于对照组（P＜0.05）；观察组术后排气时间、术后排便时间均短于对照组（P＜0.05）；观察组结石清除率为92.59%，与对照组的96.30%比较，差异无统计学意义（P＞0.05）；观察组与对照组术后4 h VAS评分比较，差异无统计学意义（P＞0.05），而观察组术后12、24 h VAS评分均低于对照组（P＜0.05）；随访1年，观察组复发率为3.70%，与对照组的7.41%比较，差异无统计学意义（P＞0.05）。结论 腹腔镜胆总管切开取石术治疗胆总管结石伴胆囊结石可获得与传统开腹手术治疗相似的效果，且可减少术中出血量，缩短术后胃肠功能恢复时间，降低术后并发症发生率，减轻术后疼痛度，利于患者的康复。
Abstract:
Objective To investigate the effect of laparoscopic choledocholithotomy on gastrointestinal function in patients with choledocholithiasis and cholecystolithiasis.Methods A total of 54 patients with choledocholithiasis and cholecystolithiasis diagnosed and treated in our hospital from January 2020 to January 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 27 cases in each group. The control group was treated with traditional open cholecystectomy and choledocholithotomy, and the observation group was treated with laparoscopic choledocholithotomy. The clinical operation index, gastric function recovery index, stone clearance rate, pain ( VAS ) score at different time periods, complication rate and recurrence rate were compared between the two groups.Results The operation time and ambulation time of the observation group were shorter than those of the control group, and the intraoperative blood loss was less than that of the control group (P<0.05). The postoperative exhaust time and postoperative defecation time in the observation group were shorter than those in the control group (P<0.05). The stone clearance rate of the observation group was 92.59%, which was compared with 96.30% of the control group, the difference was not statistically significant (P>0.05). There was no significant difference in VAS score between the observation group and the control group at 4 h after operation (P>0.05), while the VAS scores of the observation group at 12 and 24 h after operation were lower than those of the control group (P<0.05).Conclusion Laparoscopic choledocholithotomy in the treatment of choledocholithiasis with cholecystolithiasis can achieve similar results as traditional open surgery, and can reduce intraoperative blood loss, shorten postoperative gastrointestinal function recovery time, reduce the incidence of postoperative complications, reduce postoperative pain, and facilitate the rehabilitation of patients.
