摘要:
目的 研究冠状动脉粥样硬化性心脏病(CHD)心功能分级与心脏超声指标的相关性。方法 以2020年1月-2021年12月句容市人民医院确诊的207例CHD患者为研究对象,依据美国纽约心脏病协会(NYHA)心功能分级分为Ⅰ级组(55例)、Ⅱ级组(53例)、Ⅲ级组(51例)、Ⅳ级组(48例)。比较各组心脏超声指标[左心室射血分数(LVEF)、左房内径(LAD)、左室舒张末期内径(LVDd)、二尖瓣口舒张早期峰值血流速度/二尖瓣环侧壁舒张早期峰值运动速度(E/e′)],并对其相关性进行分析。结果 LVEF由高到低依次为Ⅰ级组>Ⅱ级组>Ⅲ级组>Ⅳ级组(P<0.05),LAD、LVDd、E/e′由高到低依次为Ⅳ级组>Ⅲ级组>Ⅱ级组>Ⅰ级组(P<0.05);Spearman等级相关分析显示,LVEF与心功能分级呈负相关,LAD、LVDd、E/e′与心功能分级呈正相关(P<0.05);单因素及多因素Logistic回归分析显示,LVEF、LAD、LVDd为心功能分级的独立预测因子(P<0.05)。结论 CHD患者心功能分级与心脏超声指标密切相关,其中LVEF、LAD、LVDd均为心功能分级的影响因素。
Abstract:
Objective To study the correlation between cardiac function classification and cardiac ultrasound indexes in coronary atherosclerotic heart disease (CHD).Methods A total of 207 CHD patients diagnosed in Jurong People’s Hospital from January 2020 to December 2021 were selected as the research objects. According to the New York Heart Association (NYHA) cardiac function classification, they were divided into grade Ⅰ group (55 cases), grade Ⅱ group (53 cases), grade Ⅲ group (51 cases) and grade Ⅳ group (48 cases). The echocardiography indexes [left ventricular ejection fraction (LVEF), left atrial diameter (LAD), left ventricular end-diastolic diameter (LVDd), early diastolic peak flow velocity of mitral orifice/early diastolic peak velocity of mitral annulus (E/e′)] were compared among the groups, and their correlation was analyzed.Results The LVEF index from high to low was grade Ⅰ group>grade Ⅱ group>grade Ⅲ group>grade Ⅳ group (P<0.05), LAD, LVDd, E/e′ indexes from high to low were grade Ⅳ group> grade Ⅲ group>grade Ⅱ group>grade Ⅰ group (P<0.05). Spearman rank correlation analysis showed that LVEF was negatively correlated with cardiac function classification, and LAD, LVDd and E/e′ were positively correlated with cardiac function classification (P<0.05). Univariate and multivariate Logistic regression analysis showed that LVEF, LAD and LVDd were independent predictors of cardiac function classification (P<0.05).Conclusion The cardiac function classification of CHD patients is closely related to cardiac ultrasound indexes, and LVEF, LAD and LVDd are the influencing factors of cardiac function classification.
