摘要:
目的 研究氨甲环酸联合抗生素降低侧卧位DAA入路全髋关节置换患者围术期感染的有效性。方法 选取2020年6月-2021年12月我院行侧卧位DAA入路全髋关节置换术的60例股骨颈骨折患者为研究对象，采用随机数字表法分为A组（30例）和B组（30例）。A组采用抗生素联合氨甲环酸治疗，B组采用抗生素治疗，比较两组临床手术指标、疼痛程度（VAS评分）、髋关节功能（Harris评分）、生活质量以及并发症发生率。结果 A组总失血量、术后引流量均小于B组，下床活动时间、住院时间均短于B组（P＜0.05）；A组术后1天VAS评分低于B组（P＜0.05），但A组术后3天VAS评分与B组比较，差异无统计学意义（P＞0.05）；A组术后3、6、12个月Harris评分均高于B组（P＜0.05）；A组术后3个月生活质量各维度（情感功能、意识功能、躯体功能、社会功能）评分均高于B组（P＜0.05）；A组并发症发生率为0，低于B组的13.33%（P＜0.05）。结论 侧卧位DAA入路全髋关节置换围术期应用抗生素及氨甲环酸可以减少总失血量、术后引流量，缩短下床活动时间、住院时间，减轻患者疼痛，改善髋关节功能，提高生活质量，降低并发症发生率，值得临床应用。
Abstract:
Objective To study the effectiveness of tranexamic acid combined with antibiotics in reducing perioperative infection in patients undergoing total hip arthroplasty via DAA approach in lateral position.Methods From June 2020 to December 2021, 60 patients with femoral neck fracture who underwent total hip arthroplasty through DAA approach in lateral position in our hospital were selected as the research objects. They were divided into group A (30 patients) and group B (30 patients) by random number table method. Group A was treated with antibiotics combined with tranexamic acid, and group B was treated with antibiotics. The clinical operation indexes, pain degree (VAS score), hip function (Harris score), quality of life and complication rate were compared between the two groups.Results The total blood loss and postoperative drainage volume in group A were less than those in group B, and the ambulation time and hospitalization time were shorter than those in group B (P<0.05). The VAS score of group A was lower than that of group B at 1 d after operation (P<0.05), but there was no significant difference between group A and group B at 3 d after operation (P>0.05). The Harris scores of group A were higher than those of group B at 3, 6 and 12 months after operation (P<0.05). The scores of quality of life ( emotional function, consciousness function, physical function and social function ) in group A were higher than those in group B at 3 months after operation (P<0.05). The incidence of complications in group A was 0, which was lower than 13.33% in group B (P<0.05).Conclusion The perioperative application of antibiotics and tranexamic acid in total hip arthroplasty through DAA approach in lateral position can reduce the total blood loss, postoperative drainage volume, shorten the time of getting out of bed and hospitalization, reduce the pain of patients, improve the function of hip joint, improve the quality of life and reduce the incidence of complications.
