目的 分析社区获得性肺炎病例的病原学培养结果及耐药情况,以期为临床合理使用抗生素及避免耐药性提供依据。方法 收集百色市妇幼保健院2021-2022年130例社区获得性肺炎住院病例的临床资料、病原菌培养结果以及主要病菌耐药性资料,并对其进行分析。结果 130例社区获得性肺炎患者痰培养结果共检出病原菌60株(46.15%),其中革兰氏阴性菌34株(56.67%),以肺炎克雷伯杆菌为主;革兰氏阳性菌23株(38.33%),以肺炎链球菌为主;真菌3株(5.00%),均属于假丝酵母菌类。革兰氏阳性菌和阴性菌均对万古霉素、亚胺培南等敏感,其中革兰氏阳性菌对喹诺酮类、头孢类抗生素均相对敏感,对青霉素、氨苄西林、克林霉素类药物部分耐药;革兰氏阴性菌对庆大霉素等药物部分耐药。结论 辖区内社区获得性肺炎致病菌以革兰氏阴性菌为主,其次为革兰氏阳性菌;药敏培养结果提示,对万古霉素、亚胺培南等全部敏感,对阿莫西林、左氧氟沙星、头孢曲松等药物大部分敏感,对青霉素、庆大霉素、头孢呋辛等药物大部分耐药;建议临床医生在临床中合理运用抗生素治疗社区获得性肺炎并对病原菌进行耐药性动态监测。
Abstract:
Objective To analyze the pathogenic culture results and drug resistance of community-acquired pneumonia cases, so as to provide evidence for rational use of antibiotics and avoidance of drug resistance.Methods The clinical data, pathogen culture results and drug resistance data of 130 cases of community-acquired pneumonia in Baise Maternal and Child Health Hospital from 2021 to 2022 were collected and analyzed.Results A total of 60 strains of pathogenic bacteria were detected in the sputum culture results of 130 patients with community-acquired pneumonia (46.15%), including 34 strains of gram-negative bacteria (56.67%), mainly Klebsiella pneumoniae; there were 23 strains of gram-positive bacteria (38.33%), mainly Streptococcus pneumoniae; there were 3 strains of fungi (5.00%), which were all Candida. Gram-positive bacteria and negative bacteria were sensitive to vancomycin and imipenem. Gram-positive bacteria were relatively sensitive to quinolones and cephalosporin antibiotics, and partially resistant to penicillin, ampicillin and clindamycin. Gram-negative bacteria are partially resistant to drugs such as gentamicin.Conclusion The pathogenic bacteria of community-acquired pneumonia in the jurisdiction are mainly gram-negative bacteria, followed by gram-positive bacteria; the results of drug sensitivity culture show that all of them are sensitive to vancomycin and imipenem, most of them are sensitive to amoxicillin, levofloxacin and ceftriaxone, and most of them are resistant to penicillin, gentamicin and cefuroxime. It is recommended that clinicians should rationally use antibiotics in the treatment of community-acquired pneumonia and dynamically monitor the drug resistance of pathogens.
