摘要:
目的 探讨腰硬联合麻醉与硬膜外麻醉在无痛分娩产妇中的应用效果及对患者血清皮质醇（Cor）、促肾上腺皮质激素（ACTH）、C反应蛋白（CRP）的影响。方法 选取2018年4月~2020年3月我院分娩的孕产妇300例作为研究对象，按治疗方式不同为对照组（n=100）与研究组（n=200）。对照组孕产妇予0.1%罗哌卡因+(2 μg/ml)芬太尼各5 ml的硬膜外麻醉分娩镇痛治疗，研究组予2 mg罗哌卡因+10 μg芬太尼的腰硬联合麻醉分娩镇痛治疗，比较两组孕产妇分娩阵痛前后生命体征变化、疼痛程度、血清生化指标水平、用药情况与药物不良反应情况。结果 两组各时间点HR、MAP比较，差异无统计学意义（P＞0.05）；研究组镇痛后10、20、30 minVAS评分低于对照组；研究组分娩后2 h、分娩次日Cor、ACTH、CRP水平均低于对照组；自控镇痛次数、罗哌卡因与芬太尼用量少于对照组，差异有统计学意义（P＜0.05）；两组药物不良反应发生率比较，差异无统计学意义（P＞0.05）。结论 腰硬联合麻醉分娩镇痛的镇痛效果更为显著，能够减少不良反应发生率，减轻产妇在生产过程中的疼痛感，减少产妇的应激反应，稳定产妇生命体征。
Abstract:
Objective To investigate the effect of combined spinal-epidural anesthesia on serum cortisol (Cor), adrenocorticotropic hormone (ACTH) and C-reactive protein (CRP) in painless parturient.Methods A total of 300 pregnant women who gave birth in our hospital from April 2018 to March 2020 were selected as the research objects. According to different treatment methods, they were the control group (n=100) and the study group (n=200). The pregnant women in the control group were treated with 0.1% ropivacaine + (2 μg/ml) fentanyl each 5 ml of epidural anesthesia for labor analgesia, and the study group was treated with 2 mg ropivacaine + 10 μg fentanyl for lumbar rigidity Combined with anesthesia for labor analgesia, the changes in vital signs, pain degree, serum biochemical index levels, medication and adverse drug reactions were compared between the two groups of pregnant women before and after labor labor.Results There was no significant difference in HR and MAP between the two groups at each time point (P>0.05); the VAS scores of the study group at 10, 20, and 30 min after analgesia were lower than those of the control group; and the levels of Cor, ACTH, and CRP in the study group were lower than those in the control group 2 h after delivery and the day after delivery; the number of patient-controlled analgesia and the dosage of ropivacaine and fentanyl in the study group were less than those in the control group, the difference was statistically significant (P<0.05); the difference in the incidence of adverse drug reactions between the two groups was not statistically significant (P＞0.05).Conclusion The analgesic effect of combined spinal-epidural anesthesia in labor analgesia is more significant, which can reduce the incidence of adverse reactions, relieve the pain of the parturient during the delivery process, reduce the stress response of the parturient, and stabilize the vital signs of the parturient.
