摘要:
目的 观察温针灸联合通络止痛散治疗寒湿型腰椎间盘突出症的临床疗效。方法 选取2021年1月-2022年2月在我院诊治的82例寒湿型腰椎间盘突出症患者为研究对象，采用随机数字表法分为对照组和观察组，各组41例。对照组采用温针灸治疗，观察组在对照组基础上联合通络止痛散治疗，比较两组临床疗效、中医证候评分（疼痛、下肢麻木、活动受限）、疼痛（VAS）评分、腰椎功能障碍指数（ODI）、腰椎功能评分（JOA）、复发率及不良反应发生率。结果 观察组治疗总有效率（95.12%）高于对照组（80.49%）（P＜0.05）；治疗后，两组疼痛、下肢麻木、活动受限评分均低于治疗前，且观察组低于对照组（P＜0.05）；治疗后，两组VAS评分均低于治疗前，且观察组低于对照组（P＜0.05）；治疗后，两组ODI评分均低于治疗前，JOA评分均高于治疗前，且观察组ODI评分低于对照组，JOA评分高于对照组（P＜0.05）；随访3个月，观察组复发率（7.32%）低于对照组（17.07%）（P＜0.05）；观察组不良反应发生率与对照组比较，差异无统计学意义（P＞0.05）。结论 温针灸联合通络止痛散治疗寒湿型腰椎间盘突出症效果确切，可提高临床疗效，减轻患者中医证候评分，改善腰椎功能，降低复发率，且不增加不良反应，具有良好的应用安全性。
Abstract:
Objective To observe the clinical efficacy of warm acupuncture combined with Tongluo Zhitong powder in the treatment of lumbar disc herniation of cold-dampness type.Methods From January 2021 to February 2022, 82 patients with lumbar disc herniation of cold-dampness type diagnosed and treated in our hospital were selected as the research objects. They were divided into control group (41 cases) and observation group (41 cases) by random number table method. The control group was treated with warm acupuncture, and the observation group was treated with Tongluo Zhitong powder on the basis of the control group. The clinical efficacy, TCM syndrome score (pain, lower limb numbness, activity limitation), pain (VAS) score, lumbar dysfunction index (ODI), lumbar function score (JOA), recurrence rate and incidence of adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.12%, which was higher than 80.49% in the control group (P<0.05). After treatment, the scores of pain, lower limb numbness and activity limitation in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P<0.05). The VAS score of the two groups after treatment was lower than those before treatment, and that in the observation group was lower than that in the control group (P<0.05). After treatment, the ODI scores of the two groups were lower than those before treatment, and the JOA scores were higher than those before treatment, while the ODI score of the observation group was lower than that of the control group, and the JOA score was higher than that of the control group (P<0.05). After 3 months of follow-up, the recurrence rate of the observation group was 7.32%, which was lower than 17.07% of the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the observation group and the control group (P>0.05).Conclusion Warm acupuncture combined with Tongluo Zhitong powder is effective in the treatment of lumbar disc herniation of cold-dampness type, which can improve the clinical efficacy, reduce the TCM syndrome score, improve the lumbar function, reduce the recurrence rate, and do not increase adverse reactions, with good application safety.
