摘要:
急性心肌梗死是全球第一位死亡原因,而其中最为常见的是急性前壁心肌梗死,由于梗死发生的血管即前降支会为左心室心肌供应约50%的血液,因此患者死亡率比其他部位的梗死更高。对于此类患者,尽快确保再灌注、缩小心肌缺血的范围显得尤为重要。然而,经皮冠状动脉介入治疗(PCI)作为目前国际上公认的首选治疗与诊断方法之一,在过去数年中并未能有效降低急性心肌梗死患者的死亡率及并发症的发生率。其原因是在再灌注过程中损伤非但没有恢复,反而会进一步恶化,即发生缺血再灌注损伤(MIRI)。而利用重组人脑利钠肽的相关功能,如扩展动静脉、减轻心肌细胞自由基氧化损伤等,能够提高急性前壁心肌梗死患者心肌再灌注水平、改善患者预后,其在临床上也越来越广泛的应用于急性心肌梗死的治疗。本文以心肌再灌注损伤发生机制为切入点,综述重组人脑利钠肽避免或者减轻再灌注损伤水平的作用机制,为临床治疗急性前壁心肌梗死介入术后心肌再灌注损伤提供参考。
Abstract:
Acute myocardial infarction is the first cause of death in the world, and the most common one is acute anterior myocardial infarction. Because the blood vessel of infarction, the anterior descending branch, will supply about 50 % of the blood to the left ventricular myocardium, the mortality rate of patients is higher than that of other parts. For such patients, it is particularly important to ensure reperfusion and reduce the scope of myocardial ischemia as soon as possible. However, percutaneous coronary intervention (PCI), as one of the internationally recognized preferred treatment and diagnosis methods, has not been able to effectively reduce the mortality and incidence of complications in patients with acute myocardial infarction in the past few years. The reason is that the injury does not recover during reperfusion, but will deteriorate further, that is myocardial ischemia-reperfusion injury (MIRI). The use of recombinant human brain natriuretic peptide related functions, such as the expansion of arteries and veins, reducing myocardial cell free radical oxidative damage, etc., can improve the level of myocardial reperfusion in patients with acute anterior myocardial infarction and improve the prognosis of patients. It is also more and more widely used in the treatment of acute myocardial infarction. Based on the mechanism of myocardial reperfusion injury, this paper reviews the mechanism of recombinant human brain natriuretic peptide to avoid or reduce the level of reperfusion injury, and provides reference for clinical treatment of myocardial reperfusion injury after interventional therapy for acute anterior myocardial infarction.
