摘要:
目的 探讨血清血管生成素2（Ang-2）、血管内皮生长因子（VEGF）、 畸胎瘤细胞衍生生长因子（PCDGF）及癌胚抗原（CEA）、鳞状细胞癌抗原（SCC）与非小细胞肺癌临床病理参数的关系，判断其在非小细胞肺癌联合诊断中的价值。方法 收集2017年9月~2018年4月就诊于安医大附属巢湖医院呼吸内科，经病理确诊的48例原发性非小细胞肺癌患者，收集患者临床病理资料，年龄、性别、吸烟、病理、分期、转移程度，并采用ELISA 法检测48例非小细胞肺癌患者血清中Ang-2、VEGF、PCDGF含量及电化学发光检测CEA、SCC的含量。诊断效能评价指标用ROC曲线分析，以敏感度、特异度、正确率表示。结果在鳞癌患者中Ang-2含量高于腺癌患者（P＜0.05）；PCDGF在年龄≤60岁的患者中的水平低于年龄＞60岁的患者（P＜0.01）；对于局部转移的非小细胞肺癌患者血清VEGF含量高于远处转移患者（P＜0.05）；Ang-2+PCDGF联合检测诊断肺鳞癌灵敏度和正确率均为最高，分别为82.76%和75.00%。结论 Ang-2、PCDGF联合检测对于肺鳞癌的诊断价值最高，VEGF能够更好预测非小细胞肺癌局部转移，CEA仍然是腺癌的最敏感的标记物。
Abstract:
Abstract：Objective To investigate serum angiopoietin 2 (Ang-2), vascular endothelial growth factor (VEGF), teratoma cell-derived growth factor (PCDGF) and carcinoembryonic antigen (CEA), squamous cell carcinoma antigen (SCC) and non-small The relationship between clinical pathological parameters of lung cancer and its value in the combined diagnosis of non-small cell lung cancer.Methods From September 2017 to April 2018, we received 48 patients with primary non-small cell lung cancer diagnosed by pathology in the Department of Respiratory Medicine, Chaohu Hospital, Anyang University. The clinical and pathological data were collected. Age, sex, smoking, pathology, The levels of Ang-2, VEGF and PCDGF in serum of 48 patients with non-small cell lung cancer were detected by ELISA. The contents of CEA and SCC were detected by electrochemiluminescence. The diagnostic efficacy evaluation index was analyzed by ROC curve and expressed by sensitivity, specificity and correct rate.Results The level of Ang-2 was higher in patients with squamous cell carcinoma than in adenocarcinoma patients(P<0.05); the level of PCDGF in patients aged ≤60 years was lower than that in patients >60 years old (P<0.01); the serum VEGF level in patients with locally metastatic non-small cell lung cancer was higher than patients with distant metastasis (P<0.05); the sensitivity and correct rate of Ang-2+PCDGF combined detection for lung squamous cell carcinoma were the highest, 82.76% and 75.00%, respectively.Conclusion The combined detection of Ang-2 and PCDGF has the highest diagnostic value for lung squamous cell carcinoma. VEGF can better predict the local metastasis of non-small cell lung cancer. CEA is still the most sensitive marker for adenocarcinoma.
