摘要:
目的 探讨慢性阻塞性肺疾病(COPD)患者血清HIF-1α、caspase-3、Hcy水平与认知障碍的相关性及临床意义。方法 收集简阳市人民医院2015年5月~2020年5月的COPD患者144例,按是否存在认知功能障碍分组,将存在认知障碍的70例患者作为认知障碍组,不存在认知障碍的74例患者作为认知正常组,并选取同期门诊健康体检者60例作为健康对照组,测定三组血清HIF-1α、caspase-3、Hcy含量水平,分析其与认知障碍的关系及血清水平预测认知障碍的受试者工作特征(ROC)曲线。结果 认知正常组、障碍组患者血清HIF-1α、caspase-3、Hcy水平高于健康对照组,认知障碍组患者血清HIF-1α、caspase-3、Hcy水平高于认知正常组,三组患者两两比较差异有统计学意义(P<0.05);认知障碍组MoCA评分低于健康对照组(P<0.05),与血清HIF-1α、caspase-3、Hcy水平呈负相关(r=-0.425、-5.453、-4.112,P<0.05);血清HIF-1α、caspase-3、Hcy暴露水平预测认知障碍的ROC曲线下面积分别为0.830(95%CI:0.654~0.921)、0.813(95%CI:0.669~0.901)、0.821(95%CI:0.687~0.899)。结论 COPD患者血清HIF-1α、caspase-3、Hcy水平能反应认知功能损伤的严重程度,在认知障碍产生过程中发挥着重要作用,可作为COPD患者认知损伤的筛查评估指标。
Abstract:
Objective To investigate the correlation and clinical significance of serum HIF-1α, caspase-3, Hcy levels and cognitive impairment in patients with chronic obstructive pulmonary disease (COPD).Methods A total of 144 COPD patients from Jianyang People’s Hospital from May 2015 to May 2020 were collected.According to the presence or absence of cognitive impairment, 70 patients with cognitive impairment are regarded as the cognitive impairment group, and 74 patients without cognitive impairment are regarded as the cognitive normal group and selected 60 healthy outpatients in the same period as the healthy control group.The serum levels of HIF-1α, caspase-3, and Hcy in the three groups were determined, and the relationship between them and cognitive impairment was analyzed and the serum level predicted the receiver operating characteristic (ROC) curve of cognitive impairment.Results The serum HIF-1α, caspase-3, and Hcy levels of the cognitively normal group and the impaired group were higher than those of the healthy control group. The serum HIF-1α, caspase-3 and Hcy levels of the cognitively impaired group were higher than those of the cognitively normal group,the difference between the two groups of patients was statistically significant (P<0.05);The MoCA score of the cognitive impairment group was lower than that of the healthy control group (P<0.05),it was negatively correlated with serum HIF-1α, caspase-3 and Hcy levels (r=-0.425, -5.453, -4.112, P<0.05);The area under the ROC curve of serum HIF-1α, caspase-3 and Hcy exposure levels predicting cognitive impairment are 0.830 (95%CI: 0.654~0.921), 0.813 (95%CI: 0.669~0.901),0.821(95%CI: 0.687~0.899).Conclusion The serum levels of HIF-1α, caspase-3 and Hcy in COPD patients can reflect the severity of cognitive impairment, play an important role in the process of cognitive impairment, and can be used as screening and evaluation indicators for cognitive impairment in COPD patients.
