摘要:
目的 分析2018-2020年我院分离金黄色葡萄球菌(SA)临床分布情况及细菌耐药性特点。方法 收集2018-2020年我院SA阳性患者的临床资料,对SA的特点和耐药性进行回顾性分析。结果 2018-2020年我院检出SA共1449株,其中耐甲氧西林金黄色葡萄球菌(MRSA)417株,占28.78%;对甲氧西林敏感的金黄色葡萄球菌(MSSA)1032株,占71.22%。标本中检出SA最多的是分泌物,其次是痰液、血液、脓液、引流液及尿液等,而MRSA在脓液、引流液和痰中分布较多。科室中检出SA最多的是皮肤科,其次是儿科、内分泌科、神经外科,而MRSA在儿科、烧伤科、神经外科检出率较高。SA对青霉素的耐药率为87.70%,红霉素、苯唑西林、克林霉素、环丙沙星、左氧沙星、复方新诺明的耐药率分别为39.90%、27.40%、19.60%、7.10%、6.70%、5.40%,且MRSA的耐药率几乎都比MSSA高。结论 我院分离的SA感染类型多样化,MRSA对多种抗生素耐药,应加强临床感染监控,在治疗SA感染时应根据药敏结果合理使用抗生素。
Abstract:
Objective To analyze the distribution of Staphylococcus aureus (SA) isolated in our hospital from 2018 to 2020 and the characteristics of antibiotics resistance.Methods The clinical data of SA-positive patients in our hospital from 2018 to 2020 were collected, and the characteristics and drug resistance of SA were retrospectively analyzed.Results A total of 1449 strains of SA were detected in our hospital from 2018 to 2019, including 417 strains of methicillin-resistant Staphylococcus aureus (MRSA), accounting for 28.78%. Methicillin-sensitive Staphylococcus aureus (MSSA) 1032 strains, accounting for 71.22%. The secretions were the most detected SA in the specimens, followed by sputum, blood, pus, drainage fluid and urine, while MRSA was widely distributed in pus, drainage fluid and sputum. The most detected SA was in the department of dermatology, followed by department of pediatrics, department of endocrinology and department of neurosurgery, while MRSA had a high detection rate in department of pediatrics, department of burn injury and department of neurosurgery. The resistance rate of SA to penicillin was 87.70%. The resistance rates of erythromycin, oxacillin, clindamycin, ciprofloxacin, levofloxacin and cotrimoxazole were 39.90%, 27.40%, 19.60%, 7.10%, 6.70% and 5.40%, respectively. The resistance rate of MRSA was almost higher than that of MSSA.Conclusion The types of SA infection isolated in our hospital are diversified. MRSA is resistant to multiple antibiotics. Clinical infection monitoring should be strengthened, and antibiotics should be rationally used according to drug sensitivity results in the treatment of SA infection.
