摘要:
目的 探讨硼替佐米为基础化疗对多发性骨髓瘤患者认知功能和情绪的影响。方法 选取2020年11月-2021年7月安徽医科大学第一附属医院收治的多发性骨髓瘤患者130例作为研究对象,根据是否使用硼替佐米化疗分为未化疗组(31例)和化疗组(99例),其中化疗组根据使用硼替佐米化疗疗程分为化疗≤4疗程组(58例)及化疗>4疗程组(41例)。比较各组蒙特利尔量表(MoCA)评分、汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分及血清肿瘤坏死因子-α(TNF-α)的水平,并分析化疗组TNF-α水平与MoCA、HAMA、HAMD评分的相关性。结果 化疗≤4疗程组、化疗>4疗程组MoCA、HAMA、HAMD评分低于未化疗组,血清TNF-α水平高于未化疗组,差异有统计学意义(P<0.05);化疗≤4疗程组MoCA评分高于化疗>4疗程组,血清TNF-α水平低于化疗>4疗程组,差异有统计学意义(P<0.05);两组HAMA和HAMD评分比较,差异无统计学意义(P>0.05);Pearson相关性分析显示,化疗组血清TNF-α水平与MoCA评分呈负相关(P<0.05),与HAMA、HAMD评分无相关性(P>0.05);HAMA评分与HAMD评分呈正相关(P<0.05)。结论 硼替佐米为基础化疗会影响多发性骨髓瘤患者认知功能,但不会影响患者情绪,且多发性骨髓瘤患者的认知功能与血清TNF-α水平密切相关。
Abstract:
Objective To investigate the effect of bortezomib-based chemotherapy on cognitive function and emotion in patients with multiple myeloma.Methods A total of 130 patients with multiple myeloma admitted to the First Affiliated Hospital of Anhui Medical University from November 2020 to July 2021 were selected as the research objects. They were divided into the non-chemotherapy group (31 cases) and the chemotherapy group (99 cases) according to whether or not to use bortezomib chemotherapy. The chemotherapy group was divided into the chemotherapy≤4 courses group (58 cases) and the chemotherapy>4 courses group (41 cases) according to the course of bortezomib chemotherapy. The Montreal Cognitive Assessment Scale (MoCA), Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD) and serum tumor necrosis factor-α (TNF-α) levels were compared among the groups, and the correlation between TNF-α level and MoCA, HAMA and HAMD scores in the chemotherapy group was analyzed.Results The scores of MoCA, HAMA and HAMD in chemotherapy≤4 courses group and chemotherapy>4 courses group were lower than those in non-chemotherapy group, and the serum TNF-α level was higher than that in non-chemotherapy group, the difference was statistically significant(P<0.05). The MoCA score of chemotherapy≤4 courses group was higher than that of chemotherapy>4 courses group, and the serum TNF-α level was lower than that of chemotherapy>4 courses group, the difference was statistically significant (P<0.05); there was no significant difference in HAMA and HAMD scores between the two groups (P>0.05). Pearson correlation analysis showed that the serum TNF-α level in the chemotherapy group was negatively correlated with the MoCA score (P<0.05), there was no correlation with HAMA and HAMD scores (P>0.05);HAMA score was positively correlated with HAMD score(P<0.05).Conclusion Bortezomib-based chemotherapy can affect the cognitive function of patients with multiple myeloma, but does not affect the mood of patients. The cognitive function of patients with multiple myeloma is correlated with the level of serum TNF-α.
