摘要:
目的 探讨应用直接前侧（DAA）入路空心钉联合钢板治疗股骨颈骨折的疗效。方法 回顾性分析2013年1月~2018年8月我院收治的21例GardenⅣ型股骨颈骨折患者临床资料，均行DAA入路空心钉联合股骨颈内侧钢板固定治疗，分析术后Garden指数、Harris评分及并发症发生情况。结果 共21例股骨颈骨折患者，手术时间70~150 min，平均手术时间（91.66±23.17）min；出血量150~500 ml，平均出血量（217.31±32.61）ml；术后Garden指数：Ⅰ级16例，Ⅱ级4例，Ⅲ级1例，均6月内达到骨愈合，愈合率为100.00%。Harris评分：优10例（46.62%），良7例（33.33%），可3例（14.29%），差1例（4.76%），优良率为80.95%（17/21）。术后1年复查，股骨头缺血性坏死1例，发生率4.76%（1/21）。结论 空心钉联合钢板固定治疗GardenⅣ型股骨颈骨折疗效确切，可获得稳定固定，骨折复位良好，术后并发症发生率低。
Abstract:
Objective To investigate the effect of direct anterior (DAA) approach with hollow nails combined with steel plates in the treatment of femoral neck fractures.Methods Retrospective analysis of clinical data of 21 patients with Garden IV femoral neck fractures admitted in our hospital from January 2013 to August 2018. All patients underwent DAA approach hollow nail oblique combined with medial femoral neck plate fixation. The postoperative Garden index, Harris score, and complications were analyzed.Results A total of 21 patients with femoral neck fractures had an operation time of 70 to 150 minutes with an average operation time of (91.66±23.17) minutes; a bleeding volume of 150 to 500 ml with an average bleeding volume of (217.31±32.61) ml; the postoperative Garden index: Grade Ⅰ16 cases, grade Ⅱ 4 cases, grade Ⅲ in 1 case, all achieved bone healing within 6 months, the healing rate was 100.00%. Harris score: 10 cases were excellent (46.62%), 7 cases were good (33.33%), 3 cases were fair (14.29%), 1 case was poor (4.76%), and the excellent and good rate was 80.95% (17/21). One year after surgery, one case of ischemic necrosis of the femoral head occurred in 4.76% (1/21).Conclusion Hollow nail combined with steel plate fixation for Garden Ⅳ femoral neck fractures is effective, stable stabilization can be obtained, fracture reduction is good, and the incidence of postoperative complications is low.
