摘要:
目的 分析鼻内镜手术治疗慢性鼻窦炎的临床效果及其与临床分型的关系。方法 选取2017年10月~2019年10月在我院诊治的128例慢性鼻窦炎患者为研究对象，采用随机数字表法分为对照组和观察组,各64例。对照组采用传统根治术治疗，观察组采用鼻内镜手术治疗，比较两组患者临床治疗效果、治疗前后鼻腔鼻窦结局测试-20量表（SNOT-20）评分、临床症状评分、不同分型复发情况以及观察组不同分型临床治疗效果。结果 观察组临床治疗总有效率为92.18%，高于对照组的81.25%，差异有统计学意义（P＜0.05）；治疗后两组SNOT-20、临床症状评分均低于治疗前，且观察组低于对照组，差异有统计学意义（P＜0.05）；随访3个月，观察组Ⅰ型、Ⅱ型、Ⅲ型复发率均低于对照组，差异有统计学意义（P＜0.05）；观察组患者Ⅰ型、Ⅱ型治疗总有效率均高于Ⅲ型, 差异有统计学意义（P＜0.05），但Ⅰ型与Ⅱ型比较，差异无统计学意义（P＞0.05），Ⅰ型治愈率高于Ⅱ型、Ⅲ型，且Ⅱ型高于Ⅲ型，差异有统计学意义（P＜0.05）。结论 鼻内镜手术治疗慢性鼻窦炎的临床效果确切，总有效率高，复发率低，且治疗效果受临床分型影响，分型越严重治愈率越低。
Abstract:
Objective To analyze the clinical effect of endoscopic sinus surgery in the treatment of chronic sinusitis and its relationship with clinical classification.Methods A total of 128 patients with chronic sinusitis who were diagnosed and treated in our hospital from October 2017 to October 2019 were selected as the research objects. They were divided into the control group and the observation group by random number table, with 64 cases in each group. The control group was treated with traditional radical resection, and the observation group was treated with endoscopic sinus surgery. The clinical treatment effects of the two groups were compared, the nasal cavity and sinus outcome test -20 scale (SNOT-20) score, clinical symptom score, and different types of recurrence before and after treatment the situation and the clinical treatment effect of different types in the observation group.Results The total effective rate of clinical treatment in the observation group was 92.18%, which was higher than 81.25% in the control group,the difference was statistically significant(P＜0.05); after treatment, the SNOT-20 and clinical symptom scores of the two groups were lower than those before treatment, and the observation group Lower than the control group, the difference was statistically significant (P＜0.05); follow-up for 3 months, the observation group’s type I, type II, and type III recurrence rates were lower than the control group, the difference was statistically significant (P＜0.05); The total effective rate of type Ⅰ and type Ⅱ treatment in the observation group was higher than that of type Ⅲ, the difference was statistically significant (P＜0.05), but the difference between type Ⅰ and type Ⅱ was not statistically significant (P＞0.05), type Ⅰ cure rate was higher than type Ⅱ and type Ⅲ, and type Ⅱ was higher than type Ⅲ, the difference was statistically significant (P＜0.05).Conclusion The clinical effect of endoscopic sinus surgery in the treatment of chronic sinusitis is definite, the total effective rate is high, the recurrence rate is low, and the treatment effect is affected by the clinical classification. The more serious the classification, the lower the cure rate.
