摘要:
目的 探讨胎膜早破新生儿脐血中PCT、IL-6、CRP对早期识别新生儿败血症(EONS)的意义。方法 选取2020年1月~9月在我院足月分娩的新生儿共90例作为研究对象,其中产妇胎膜早破大于12 h以上的足月新生儿作为实验组(40例),未发生胎膜早破的足月新生儿作为对照组(50例),其中实验组诊断为EONS为的22例作为EONS组,非EONS18例作为非EONS组;检测各组新生儿脐动脉血PCT、IL-6、CRP水平,绘制脐血PCT、IL-6、CRP诊断EONS的ROC曲线,比较各指标及其联合诊断EONS的价值。结果 实验组脐血PCT、IL-6、CRP的水平均高于对照组,差异有统计学意义(P<0.05);实验组中胎膜早破>18 h的新生儿脐血PCT、IL-6及CRP水平均高于胎膜早破≤18 h的新生儿,发生EONS的新生儿脐血PCT、IL-6、CRP水平均高于非EONS新生儿,差异均有统计学意义(P<0.05);ROC曲线显示,脐血PCT、IL-6、CRP诊断EONS的最佳界值分别为4.2 ng/ml、60.85 pg/ml、1.17 mg/L,三者单独检测的曲线下面积分别为0.949、0.886、0.826,联合检测的曲线下面积为0.972。结论 胎膜早破新生儿脐血PCT、IL-6、CRP检测在早期识别EONS中具有一定价值,联合检测时诊断的准确性更高。
Abstract:
Objective To explore the significance of PCT, IL-6 and CRP in cord blood of newborns with premature rupture of membranes in the early recognition of neonatal sepsis (EONS).Methods A total of 90 newborns who were delivered at term in our hospital from January to September 2020 were selected as the research objects. Among them, the term newborns with premature rupture of membranes greater than 12 h were selected as the experimental group (40 cases). Full-term newborns with premature rupture of membranes served as a control group (50 cases), of which 22 cases diagnosed with EONS in the experimental group were regarded as the EONS group, and 18 cases with non-EONS were regarded as the non-EONS group;Detect the levels of PCT, IL-6 and CRP in cord blood of newborns in each group, draw the ROC curve of cord blood PCT, IL-6, and CRP in the diagnosis of EOS, compare each index and the value of combined diagnosis of EOS.Results The levels of PCT, IL-6 and CRP in cord blood of the experimental group were higher than those of the control group, the difference was statistically significant (P<0.05);In the experimental group, the levels of cord blood PCT, IL-6 and CRP of newborns with premature rupture of membranes> 18 h were higher than those of newborns with premature rupture of membranes ≤18 h.The cord blood PCT, IL-6 and CRP levels of neonates with EONS were higher than those of non-EONS neonates, and the differences were statistically significant (P<0.05);The ROC curve showed that the best cut-off values of cord blood PCT, IL-6, and CRP for the diagnosis of EOS were 4.2 ng/ml, 60.85 pg/ml, and 1.17 mg/L, respectively.The area under the curve of the three independent detections were 0.949, 0.886, 0.826, and the area under the curve of the combined detection was 0.972.Conclusion The detection of PCT, IL-6 and CRP in cord blood of newborns with premature rupture of membranes has certain value in the early identification of EONS, and the diagnosis accuracy is higher when combined detection.
