摘要:
目的 观察PCI术前应用替罗非班对急性心肌梗死患者PCI术临床效果的影响。方法 选取2019年5月-2020年9月我院收治的急性心肌梗死患者90例作为研究对象，根据随机数字表法分为观察组和对照组，各45例。对照组PCI术前给予阿司匹林+硫酸氢氯吡格雷，术中给予肝素钠；观察组在对照组基础上于术前给予盐酸替罗非班。比较两组PCI术后心肌梗死溶栓（TIMI）血流分级、心肌灌注分级（TMP）、ST段回落情况、心功能评估及术后6个月主要心血管不良事件（MACE）发生情况。结果 观察组TIMI及TMPⅢ级占比高于对照组（P＜0.05）；两组TIMI及TMP0～Ⅱ级占比比较，差异无统计学意义（P＞0.05）；观察组PCI术后ST段回落率＞50%占比为87.58%，高于对照组的62.22%（P＜0.05）；观察组术后7 d LVEF高于对照组（P＜0.05）；观察组术后6个月MACE总发生率为8.89%，低于对照组的24.44%（P＜0.05）。结论 急性心肌梗死患者行PCI术前应用替罗非班可以恢复急性心肌梗死患者冠状动脉血流灌注及心肌灌注，改善患者的心脏功能，减少MACE，预后良好。
Abstract:
Objective To observe the effect of tirofiban on the clinical effect of PCI in patients with acute myocardial infarction before PCI.Methods A total of 90 patients with acute myocardial infarction admitted to our hospital from May 2019 to September 2020 were selected as the research objects. According to the random number table method, they were divided into observation group and control group, with 45 cases in each group. The control group was given aspirin+clopidogrel bisulfate before PCI and heparin sodium during PCI. The observation group was given tirofiban hydrochloride before operation on the basis of the control group. The thrombolysis in myocardial infarction (TIMI) flow grade, myocardial perfusion grade (TMP), ST segment resolution, cardiac function evaluation and major adverse cardiovascular events (MACE) 6 months after PCI were compared between the two groups.Results The proportion of TIMI and TMP Ⅲ in the observation group was higher than that in the control group(P<0.05). There was no significant difference in TIMI and TMP 0 ~ II between the two groups (P>0.05). The proportion of ST segment fall rate>50% in the observation group after PCI was 87.58%, which was higher than 62.22% in the control group(P<0.05). The LVEF at 7 days after operation of the observation group was higher than that of the control groupt (P<0.05). The total incidence of MACE at 6 months after operation in the observation group was 8.89%, which was lower than 24.44% in the control group (P<0.05).Conclusion The application of tirofiban before PCI in patients with acute myocardial infarction can restore coronary blood flow perfusion and myocardial perfusion in patients with acute myocardial infarction, improve cardiac function, reduce MACE, and have a good prognosis.
