摘要:
目的 研究卡前列素氨丁三醇联合缩宫素防治前置胎盘引起产后出血的效果。方法 选取2021年5月-2022年5月在我院诊治的64例前置胎盘产妇为研究对象，采用随机数字表法分为对照组（n=32）和观察组（n=32）。对照组以缩宫素治疗，观察组缩宫素+卡前列素氨丁三醇治疗，观察2组产后出血发生率、不同时间段出血量、凝血功能指标、不良反应及并发症发生率。结果 观察组产后出血发生率（15.63%）低于对照组（25.00%）（P＜0.05）；观察组分娩中、产后2 h、产后24 h出血量均小于对照组（P＜0.05）；观察组凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）水平均低于对照组，纤维蛋白原（FIB）高于对照组（P＜0.05）；观察组不良反应发生率与对照组比较，差异无统计学意义（P＞0.05）；观察组并发症发生率为9.38%，低于对照组的21.88%（P＜0.05）。结论 卡前列素氨丁三醇联合缩宫素可有效降低前置胎盘引起的产后出血发生率，减少产后24h内出血量，改善凝血功能指标，且无显著不良反应，值得临床加以应用。
Abstract:
Objective To study the effect of carboprost tromethamine combined with oxytocin in the prevention and treatment of postpartum hemorrhage caused by placenta previa.Methods A total of 64 pregnant women with placenta previa who were diagnosed and treated in our hospital from May 2021 to May 2022 were selected as the research objects. They were divided into control group and observation group by random number table method, with 32 pregnant women in each group. The control group was treated with oxytocin, and the observation group was treated with carboprost tromethamine on the basis of the control group. The incidence of postpartum hemorrhage, bleeding volume at different time periods, coagulation function indexes, adverse reactions and complications were compared between the two groups.Results The incidence of postpartum hemorrhage in the observation group (15.63%) was lower than that in the control group (25.00%) (P<0.05). The amount of bleeding during delivery, 2 h and 24 h after delivery in the observation group was less than that in the control group (P<0.05). The levels of prothrombin time (PT) and activated partial thromboplastin time (APTT) in the observation group were lower than those in the control group, and fibrinogen (FIB) was higher than that in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the observation group and the control group (P>0.05). The incidence of complications in the observation group was 9.38%, which was lower than 21.88% in the control group (P<0.05).Conclusion The combination of carboprost tromethamine and oxytocin can effectively reduce the incidence of within 24 hours and improve the coagulation function without significant adverse reactions, it is worthy of clinical application.
