摘要:
目的 观察阿司匹林联合氯吡格雷治疗对脑梗塞患者凝血功能及血液流变学的影响。方法 选取2019年4月-2022年4月在我院诊治的60例脑梗塞患者为研究对象，采用随机数字表法分为对照组和观察组，各组30例。对照组采用阿司匹林治疗，观察组在对照组基础上给予氯吡格雷治疗。比较两组临床疗效、凝血功能指标[纤维蛋白原（FIB）、凝血酶原时间（APTT）、凝血酶时间（TT）]、血液流变学指标（全血黏度、血浆黏度、血小板聚集率）、神经功能缺损（NIHSS）评分，以及临床不良反应发生情况。结果 观察组治疗总有效率为93.33%，高于对照组的80.00%（P＜0.05）；两组治疗后FIB、APTT、TT均低于治疗前，且观察组低于对照组（P＜0.05）；两组治疗后全血黏度、血浆黏度、血小板聚集率水平均小于对照组，且观察组小于对照组（P＜0.05）；两组治疗后NIHSS评分均低于治疗前，且观察组低于对照组（P＜0.05）；观察组不良反应发生率为10.00%，与对照组的6.67%比较，差异无统计学意义（P＞0.05）。结论 脑梗塞患者采用阿司匹林联合氯吡格雷治疗可提疗效，改善凝血功能和血液流变学指标，降低神经功能缺损评分，促进神经功能恢复，且不增加不良反应，具有良好的应用有效性。
Abstract:
Objective To observe the effect of aspirin combined with clopidogrel on coagulation function and hemorheology in patients with cerebral infarction.Methods A total of 60 patients with cerebral infarction diagnosed and treated in our hospital from April 2019 to April 2022 were selected as the research objects. They were divided into control group and observation group by random number table method, with 30 patients in each group. The control group was treated with aspirin, and the observation group was treated with clopidogrel on the basis of the control group. The clinical efficacy, coagulation function indexes [fibrinogen (FIB), prothrombin time (APTT), thrombin time (TT)], hemorheology indexes (whole blood viscosity, plasma viscosity, platelet aggregation rate), neurological deficit (NIHSS) score and clinical adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was 93.33%, which was higher than 80.00% in the control group (P<0.05). After treatment, FIB, APTT and TT in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P<0.05). After treatment, the levels of whole blood viscosity, plasma viscosity and platelet aggregation rate in the two groups were lower than those in the control group, and those in the observation group were lower than those in the control group (P<0.05). The NIHSS score of the two groups after treatment was lower than that before treatment, and that in the observation group was lower than that in the control group (P<0.05). The incidence of adverse reactions in the observation group was 10.00%, which was compared with 6.67% in the control group, the difference was not statistically significant (P>0.05).Conclusion Aspirin combined with clopidogrel in patients with cerebral infarction can improve the curative effect, improve the coagulation function and hemorheology indexes, reduce the neurological deficit score, promote the recovery of neurological function, and do not increase the adverse reactions, which has good application effectiveness.
