摘要:
目的 了解恩施民族地区农村老年人对村医参与居家养老医养结合服务的需求。方法 采用方便抽样法，于2020年10月-2021年8月调查湖北省恩施州利川市农村地区362名60岁及以上的老年人。分析不同社会人口学特征老年人的养老服务需求情况，探讨患慢性病数量、家庭关怀、抑郁症状与老年人医养结合居家养老服务需求之间的关系。结果 民族农村老年人的医养结合居家养老服务需求得分均值为（140.47±15.82）分，不同社会人口学特征与老年人的养老服务需求得无关（P＞0.05）；老年人的医养结合居家养老服务需求得分与患慢性病数量、家庭关怀得分以及抑郁得分均呈正相关关系（P＜0.05）；多元回归结果显示，患慢病数量、家庭关怀与抑郁症状均是老年人医养结合居家养老服务需求的影响因素（P＜0.05）。结论 恩施民族地区农村老年人对村医参与居家养老医养结合服务的需求较高，尚未得到充分满足，应重点关注慢性病、家庭关怀和抑郁症状等因素对其医养结合居家养老服务需求的影响。
Abstract:
Objective To study the demand for village doctors to participate in the home care combined with medical care services for the elderly in Enshi ethnic minority areas.Methods A convenient sampling method was used to investigate 362 elderly people aged 60 years and above in rural areas of Enshi City, Hubei Province from October 2020 to August 2021. The demand of elderly care services with different social demographic characteristics was analyzed, and the relationship between the number of chronic diseases, family care, depression symptoms and the demand of medical care combined with home care for the elderly was explored.Results The average score of the demand for medical care combined with home care services for the elderly in ethnic rural areas was (140.47±15.82)scores. Different socio-demographic characteristics were not related to the elderly’s demand for care services (P>0.05). There was a positive correlation between the score of the elderly’s demand for medical care combined with home care services and the number of chronic diseases, family care score and depression score (P<0.05). Multivariate regression results showed that the number of chronic diseases, family care and depressive symptoms were the influencing factors of the demand for medical care combined with home care services for the elderly (P<0.05).Conclusion The rural elderly in Enshi ethnic areas have a high demand for village doctors to participate in the home care combined with medical care services, which has not been fully met. We should pay more attention to the influence of chronic diseases, family care and depression symptoms on their demand for village doctors to participate in the home care combined with medical care services.
