摘要:
目的 探讨老年慢性肾功能衰竭患者血液透析护理中应用综合护理干预的效果。方法 选取上栗县人民医院2022年1月-2023年12月接受血液透析治疗的老年慢性肾功能衰竭患者66例，按随机数字表法将其分成研究组（n=33）和对照组（n=33）。对照组应用常规护理干预，研究组应用综合护理干预。比较两组心理状态[焦虑（HAMA）、疾病不确定感（MUIS）、抑郁（HAMD）]、生活质量（躯体疼痛、生理职能、社会功能、生理功能、总体健康、活力、情感职能及精神健康）、并发症（血管狭窄、动脉瘤、导管血栓形成及皮肤瘙痒）发生率、自我管理能力（透析态度、自护技能、透析知识水平、自护责任感）。结果 两组护理后HAMA评分、MUIS评分、HAMD评分均较护理前降低，且与对照组比较，研究组护理后HAMA评分、MUIS评分、HAMD评分更低（P＜0.05）；两组护理后生活质量各维度评分均较护理前提高，且与对照组比较，研究组护理后生活质量各维度评分更高（P＜0.05）；研究组并发症发生率较对照组降低（P＜0.05）；两组护理后自我管理能力各维度评分均较护理前提高，且与对照组比较，研究组护理后自我管理能力各维度评分更高（P＜0.05）。结论 综合护理干预在老年慢性肾功能衰竭患者血液透析护理中的应用效果确切，能够改善患者心理状态、生活质量，提高患者自我管理能力，降低患者并发症发生率。
Abstract:
Objective To explore the effect of comprehensive nursing intervention in hemodialysis nursing of elderly patients with chronic renal failure. Methods A total of 66 elderly patients with chronic renal failure who received hemodialysis in Shangli County People’s Hospital from January 2022 to December 2023 were selected and divided into study group (n=33) and control group (n=33) according to random number table method. The control group was treated with routine nursing intervention, and the study group was treated with comprehensive nursing intervention. The psychological status [anxiety (HAMA), uncertainty in illness (MUIS), depression (HAMD)], quality of life (physical pain, physiological function, social function, physiological function, general health, vitality, emotional function and mental health), incidence of complications (vascular stenosis, aneurysm, catheter thrombosis and skin itching), self-management ability (dialysis attitude, self-care skills, dialysis knowledge level, self-care responsibility) were compared between the two groups. Results The HAMA score, MUIS score and HAMD score of the two groups after nursing were lower than those before nursing, and compared with the control group, the HAMA score, MUIS score and HAMD score after nursing of the study group were lower (P<0.05). The scores of each dimension of quality of life after nursing in the two groups were higher than those before nursing, and compared with the control group, the scores of each dimension of quality of life after nursing in the study group were higher (P<0.05). The incidence of complications in the study group was lower than that in the control group (P<0.05). The scores of each dimension of self-management ability after nursing in the two groups were higher than those before nursing, and compared with the control group, the scores of each dimension of self-management ability after nursing in the study group were higher (P<0.05). Conclusion The application of comprehensive nursing intervention in hemodialysis nursing of elderly patients with chronic renal failure is effective, which can improve the psychological state and quality of life of patients, improve the self-management ability of patients, and reduce the incidence of complications.
