摘要:
目的 研究传染性单核细胞增多症血常规实验室检诊血涂片复检的临床意义。方法 选取我院自2016年8月~2019年8月2385例儿科住院患儿拟血涂片复检的血常规实验室检诊临床资料，依据复检原则检查白细胞总数形态、分类并予评估，红细胞染色情况、大小及形态变化，血小板聚集情况、分析形态并评估总数，检查是否存在血液寄生虫。结果 拟血涂片复检的2385例中符合复检原则315例，复检异常241例（76.51%），其中血液系统疾病19例（7.88%），消化系统疾病55例（22.82%），呼吸系统疾病139例（57.68%），传染病28例（11.62%）。血液系统疾病中，传染性单核细胞增多症外周血显微镜复检的形态主要表现为病态造血细胞、原始及幼稚白细胞，其中WBC：3.8×109~33.5×109，Hb：104~145 g/L，PLT：105×109~421×109，异型淋巴细胞阳性率15.56%，主要为Ⅰ、Ⅱ型。结论 传染性单核细胞增多症血常规实验室检诊血涂片复检，通过观察细胞的内部结构、形态，鉴别异常细胞，提高检验准确性，防止误诊、漏诊，提供更为直观、科学的依据。
Abstract:
Objective To study the clinical significance of blood smear re-examination in routine laboratory tests for infectious mononucleosis. Methods From August 2016 to August 2019, we selected clinical data of routine blood laboratory examinations of 2385 paediatric inpatients with blood smears, and examined the total white blood cell morphology, classification and evaluation based on the principle of re-examination erythrocyte staining, size, and morphological changes, platelet aggregation, morphology analysis, and total number assessment to check for the presence of blood parasites. Results Of the 2385 cases of blood smear re-examination, 315 cases met the re-examination principle, and 241 were abnormal (76.51%), including 19 cases of hematological diseases (7.88%), 55 cases of digestive diseases (22.82%), and respiratory system 139 cases of disease (57.68%) and 28 cases of infectious disease (11.62%). In hematological diseases, the re-examination of infectious mononucleosis in peripheral blood microscopy mainly includes pathological hematopoietic cells, primitive and immature white blood cells, of which WBC: 3.8×109 to33.5×109, Hb:104 to 145 g/ L, PLT:105×109 to 421×109, the positive rate of atypical lymphocytes is 15.56%, mainly type Ⅰ and Ⅱ.Conclusion Re-examination of blood smears in routine laboratory tests for infectious mononucleosis, by observing the internal structure and morphology of cells, identifying abnormal cells, improving test accuracy, preventing misdiagnosis and missed diagnosis, and providing a more intuitive and scientific basis.
