摘要:
目的 探讨硝苯地平联合阿司匹林对妊娠期高血压患者母婴结局的影响。方法 选择2017年11月~2019年7月于我院接受治疗的妊娠期高血压患者140例，按照随机数字表法分为对照组和观察组，各70例。对照组予以阿司匹林治疗，观察组在对照组基础上联合硝苯地平，比较两组凝血功能[活化凝血酶原时间（APTT）、凝血酶原时间（PT）、纤维蛋白原（FIB）]、血管内皮功能[内皮素-1（ET-1）、一氧化氮（NO）]及母婴结局。结果 治疗后，两组APTT、PT水平均较治疗前升高，FIB水平较治疗前降低，且观察组APTT和PT水平高于对照组[（32.52±2.79）s vs（28.55±2.36）s]、[（14.52±1.34）s vs（12.30±1.27）s]，FIB水平低于对照组[（4.52±1.09）g/L vs（5.04±1.14）g/L]，差异有统计学意义（P＜0.05）；两组ET-1水平较治疗前降低，NO水平较治疗前升高，且观察组ET-1水平低于对照组[（50.29±10.12）ng/L vs（75.45±10.60）ng/L]，NO水平高于对照组[（975.42±95.61）mmol/L vs（715.78±70.13）mmol/L]，差异有统计学意义（P＜0.05）；观察组不良母婴结局发生率为7.14%，低于对照组的22.86%，差异有统计学意义（P＜0.05）。结论 硝苯地平联合阿司匹林治疗妊娠期高血压患者，可有效改善患者凝血、血管内皮功能及母婴结局。
Abstract:
Objective To investigate the effect of nifedipine combined with aspirin on maternal and infant outcomes in patients with gestational hypertension.Methods A total of 140 patients with gestational hypertension who were treated in our hospital from November 2017 to July 2019 were selected and divided into a control group and an observation group according to the random number table method, with 70 cases each. The control group was treated with aspirin, and the observation group was combined with nifedipine on the basis of the control group, and the coagulation function [activated thrombin time (APTT), prothrombin time (PT), fibrinogen (FIB)], blood vessels of the two groups endothelial function [Endothelin-1 (ET-1), nitric oxide (NO)] and maternal and child outcomes.Results After treatment, the levels of APTT and PT in the two groups were higher than before treatment, and the FIB level was lower than before treatment. The APTT and PT levels in the observation group were higher than those in the control group [(32.52±2.79) s vs (28.55±2.36) s], [(14.52±1.34) s vs (12.30±1.27) s], FIB level was lower than the control group [(4.52±1.09) g / L vs (5.04±1.14) g/L], the difference was statistically significant (P<0.05); ET-1 levels in both groups were lower than before treatment, NO levels were higher than before treatment, and ET-1 levels in observation group were lower than those in control group [(50.29±10.12) ng/L vs (75.45±10.60) ng/L], NO level was higher than the control group [(975.42±95.61) mmol/L vs (715.78±70.13) mmol/L], the difference was statistically significant (P<0.05); the incidence of adverse maternal and infant outcomes in the observation group was 7.14%, which was lower than 22.86% in the control group,the difference was statistically significant(P<0.05).Conclusion Nifedipine combined with aspirin can effectively improve blood coagulation, vascular endothelial function and maternal and infant outcomes in patients with gestational hypertension.
