摘要:
目的 研究为期12周的有氧运动对维持性血液透析患者矿物质代谢、睡眠质量及疲劳度的影响。方法 于2020年4~8月选取安徽医科大学附属巢湖医院血液净化中心维持性血液透析患者50例,采用随机数字表法分为运动组(25例)和对照组(25例)。对照组仅接受常规血液透析治疗,运动组在常规血液透析治疗时行踏车运动,比较两组一般临床资料,运动前和运动后12周实验室指标(血红蛋白、白蛋白、hs-CRP、血钙、血磷、25羟维生素D、及PTH)及睡眠质量和疲劳度。结果 两组性别、年龄、透析龄及病因比较,差异无统计学意义(P>0.05);运动后,运动组hs-CRP和血磷低于对照组,差异有统计学意义(P<0.05);但运动组血红蛋白、白蛋白、血钙、PTH及25(OH)D与对照组比较,差异无统计学意义(P>0.05)。运动组PSQI和FS-14评分低于对照组[(7.91±2.11)分vs(9.84±3.56)分]、[(8.48±1.05)分vs(9.72±1.51)分],差异有统计学意义(P<0.05)。结论 血液透析中有氧运动可改善维持性血液透析患者的hs-CRP、血磷、睡眠质量及疲劳度,但对血红蛋白、白蛋白、血钙、PTH及25羟维生素D没有明确影响。
Abstract:
Objective To study the effects of 12-week aerobic exercise on mineral metabolism, sleep quality and fatigue in maintenance hemodialysis (MHD) patients.Methods From April to August in 2020, 50 patients with MHD from the Blood Purification Center of Chaohu Hospital Affiliated to Anhui Medical University were selected and divided into exercise group (25 cases) and control group (25 cases) by random number table. The control group received only conventional hemodialysis treatment, and the exercise group used treadmill exercise during conventional hemodialysis treatment. The general clinical data of the two groups were compared,laboratory indicators (hemoglobin, albumin, hs-CRP, blood calcium, blood phosphorus, 25-hydroxyvitamin D, and PTH) before exercise and 12 weeks after exercise, sleep quality and fatigue.Results There was no significant difference in gender, age, dialysis age and etiology between the two groups (P>0.05); after exercise, the hs-CRP and blood phosphorus of the exercise group were lower than those of the control group, the difference was statistically significant (P<0.05); However, there was no statistically significant difference in hemoglobin, albumin, blood calcium, PTH and 25(OH)D between the exercise group and the control group (P>0.05). The PSQI and FS-14 scores of the exercise group were lower than the control group [(7.91±2.11) points vs (9.84±3.56) points], [(8.48±1.05) points vs (9.72±1.51) points], the difference was statistically significant ( P<0.05).Conclusion Aerobic exercise during hemodialysis can improve hs-CRP, blood phosphorus, sleep quality and fatigue in patients with MHD, but has no clear effect on hemoglobin, albumin, blood calcium, PTH and 25(OH)D.
