摘要:
目的 探讨盐酸氨溴索联合N-乙酰半胱氨酸（NAC）治疗老年慢性阻塞性肺病（COPD）的效果及对患者血清血管粘附蛋白-1（VAP-1）、分泌型卷曲相关蛋白1（SFRP-1）、嗜酸粒细胞阳离子蛋白（ECP）水平的影响。方法 选取2019年1月~2020年1月我院收治的125例老年COPD患者，按随机数字表法分为对照组62例与观察组63例。对照组给予盐酸氨溴索治疗，观察组在对照组基础上给予NAC治疗，比较两组治疗疗效、血清VAP-1、SFRP-1、ECP、气流受限指标第1秒用力呼气容积占预计值百分比（FEV1%pred）、肺活量占预计值百分比（FVC%pred）、FEV1/FVC及不良反应。结果 观察组总有效率为96.83%，高于对照组的83.87%，差异有统计学意义（P＜0.05）；观察组血清VAP-1、SFRP-1、ECP水平低于对照组，差异有统计学意义（P＜0.05）；观察组FEV1%pred、FVC%pred、FEV1/FVC高于对照组，差异有统计学意义（P＜0.05）；两组不良反应发生率比较，差异无统计学意义（P＞0.05）。结论 盐酸氨溴索联合NAC治疗老年COPD疗效确切，能够降低VAP-1、SFRP-1、ECP含量，改善气流受限症状。
Abstract:
Objective To investigate the effect of ambroxol hydrochloride combined with N-acetylcysteine (NAC) in the treatment of chronic obstructive pulmonary disease (COPD) in the elderly and its effect on the serum vascular adhesion protein-1 (VAP-1) and secreted frizzled-related protein 1 (SFRP-1), the influence of eosinophil cationic protein (ECP) levels.Methods A total of 125 elderly COPD patients admitted to our hospital from January 2019 to January 2020 were selected and divided into 62 cases in the control group and 63 cases in the observation group according to the random number table method.The control group was treated with ambroxol hydrochloride, and the observation group was treated with NAC on the basis of the control group.To compare the therapeutic effects of the two groups, serum VAP-1, SFRP-1, ECP, airflow limitation indicators, forced expiratory volume in the first second as a percentage of predicted value (FEV1%pred), vital capacity as a percentage of predicted value (FVC%pred), FEV1/FVC and adverse reactions.Results The total effective rate of the observation group was 96.83%, which was higher than 83.87% of the control group,the difference was statistically significant (P<0.05);The levels of serum VAP-1, SFRP-1 and ECP in the observation group were lower than those in the control group,the difference was statistically significant (P<0.05);FEV1%pred, FVC%pred, FEV1/FVC in the observation group were higher than those in the control group,the difference was statistically significant (P<0.05);There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Ambroxol hydrochloride combined with NAC has a definite effect in the treatment of elderly COPD, which can reduce the content of VAP-1, SFRP-1 and ECP, and improve the symptoms of airflow limitation.
