摘要:
目的 探究具有表皮生长因子样和Follistin样结构域的跨膜蛋白TMEFF2在子宫内膜癌(EC)组织中的表达和基因甲基化状态,以及与患者临床病理特征的关系。方法 利用美国公共癌症基因数据库(TCGA)评价TMEFF2在EC发生发展中的作用。选取2018年1月~2021年3月在我院接受手术治疗的EC组织117例,癌前病变组织95例,正常子宫内膜组织90例。经免疫组化法检测组织TMEFF2蛋白表达和定位;经巢式甲基化特异性PCR(MSP)法检测组织TMEFF2基因甲基化水平。分析TMEFF2基因甲基化状态、蛋白表达与EC患者临床病理特征的关系。结果 基于TCGA资料,原发性CESC组织中TMEFF2表达低于正常组织,但是基因启动子甲基化水平高于正常组织(P<0.05),此外低表达亚组患者中位无病生存时间和总生存时间均长于高表达亚组患者(P<0.05);在临床研究中,TMEFF2蛋白主要染色于组织细胞膜和细胞质,细胞核也偶有染色;与正常组宫内膜组和癌前病变组相比,EC组织中TMEFF2蛋白阳性表达率降低,同时基因甲基化率升高(P<0.05);另外癌前病变组织中TMEFF2蛋白阳性表达率亦高于正常子宫内膜组,且基因高甲基化率低于正常子宫内膜组(P<0.05);81例TMEFF2基因高甲基化组织中TMEFF2蛋白阳性表达率低于非甲基化组织[4.94%(4/81)vs 83.33%(30/36)],差异有统计学意义(P<0.05);EC组织中TMEFF2基因高甲基化与FIGO分期Ⅲ~Ⅳ期、淋巴结转移、肌层浸润≥1/2有关,同时TMEFF2蛋白阳性表达亦与FIGO分期Ⅲ~Ⅳ期、肌层浸润≥1/2有关(P<0.05)。结论 EC组织中普遍存在TMEFF2蛋白表达缺失以及TMEF22基因高甲基化状态,而且TMEFF2基因启动子甲基化可能是导致EC组织中TMEFF2蛋白表达降低的重要原因之一。
Abstract:
Objective To explore the expression and gene methylation status of TMEFF2, a transmembrane protein with epidermal growth factor-like and Follistin-like domains, in endometrial carcinoma (EC) tissues, and its relationship with the clinicopathological characteristics of patients.Methods U.S. Public Cancer Gene Database (TCGA) was used to evaluate the role of TMEFF2 in the occurrence and development of EC.117 cases of EC tissues that underwent surgical treatment in our hospital from January 2018 to March 2021 were selected.There were 95 cases of precancerous tissues and 90 cases of normal endometrial tissues.The expression and localization of TMEFF2 protein in tissues were detected by immunohistochemistry; the methylation level of TMEFF2 gene in tissues was detected by nested methylation-specific PCR (MSP) method.Analyze the relationship between TMEFF2 gene methylation status, protein expression and clinicopathological characteristics of EC patients.Results Based on TCGA data, the expression of TMEFF2 in primary CESC tissues was lower than that in normal tissues, but the gene promoter methylation level was higher than that in normal tissues (P<0.05).In addition, the median disease-free survival time and overall survival time of patients in the low expression subgroup were longer than those in the high expression subgroup (P<0.05);In clinical studies, TMEFF2 protein was mainly stained on the tissue cell membrane and cytoplasm, and the nucleus was also occasionally stained;Compared with the normal group, the endometrium group and the precancerous lesion group, the positive expression rate of TMEFF2 protein in EC tissues decreased, and the gene methylation rate increased (P<0.05);In addition, the positive expression rate of TMEFF2 protein in precancerous lesions was also higher than that in the normal endometrial group, and the gene hypermethylation rate was lower than that in the normal endometrial group (P<0.05);The positive expression rate of TMEFF2 protein in 81 TMEFF2 gene hypermethylated tissues was lower than that in non-methylated tissues [4.94% (4/81) vs 83.33% (30/36)], the difference was statistically significant (P<0.05);TMEFF2 gene hypermethylation in EC tissues was related to FIGO stage Ⅲ~Ⅳ, lymph node metastasis, and muscle invasion ≥ 1/2. At the same time, the positive expression of TMEFF2 protein was also related to FIGO stage Ⅲ~Ⅳ, muscle invasion ≥ 1/2 (P<0.05).Conclusion TMEFF2 protein expression loss and TMEF22 gene hypermethylation are common in EC tissues, and TMEFF2 gene promoter methylation may be one of the important reasons for the decrease of TMEFF2 protein expression in EC tissues.
