摘要:
目的 探讨子宫内膜癌组织中微小RNA-143(miR-143)的表达及与预后的相关性。方法 选取2017年1月-2018年7月我院收治的82例子宫内膜癌患者作为研究组,另选取50例同时期住院的子宫肌瘤患者作为对照组,采用实时荧光定量聚合酶链反应(PCR)检测受试者组织miR-143表达水平。根据存活情况将研究组分为死亡组、存活组,并采用PCR检测各组miR-143表达水平。绘制子宫内膜癌患者工作特征曲线(ROC)判断发生死亡时组织miR-143水平最佳截断值,根据最佳截断值将子宫内膜癌患者分为miR-143高表达组和低表达组,分析临床特征与miR-143表达的关系。随访3年,记录生存率,并分析组织miR-143表达与预后的相关性。结果 研究组miR-143表达水平低于对照组(P<0.05),且研究组中死亡者癌组织及癌旁组织miR-143表达水平低于存活组(P<0.05);ROC曲线分析显示,子宫内膜癌患者癌组织最佳截断值为0.510,曲线下面积为0.749,灵敏度为62.71%,特异度为80.21%,95%CI为1.320~3.440;不同病理分期、淋巴结转移、分化程度间miR-143表达水平比较,差异有统计学意义(P<0.05);而不同年龄、绝经状态、肿瘤直径、肌层浸润状态间miR-143表达水平比较,差异无统计学意义(P>0.05);随访3年,研究组患者癌组织miR-143高表达生存率为60.00%,高于miR-143低表达的38.46%,差异有统计学意义(P<0.05)。结论 子宫内膜癌患者癌组织中miR-143表达水平降低,其低表达对子宫内膜癌患者预后有明显影响,临床可将miR-143表达作为子宫内膜癌预后的评估指标。
Abstract:
Objective To investigate the expression of microRNA-143 (miR-143) in endometrial carcinoma and its correlation with prognosis.Methods A total of 82 patients with endometrial cancer admitted to our hospital from January 2017 to July 2018 were selected as the study group, and 50 patients with uterine fibroids admitted in the same period were selected as the control group. The expression level of miR-143 in the subjects was detected by real-time fluorescent quantitative polymerase chain reaction (PCR). According to the survival situation, the study group was divided into death group and survival group, and the expression level of miR-143 in each group was detected by PCR. The ROC curve was drawn to determine the optimal cut-off value of miR-143 level in tissues at death. According to the optimal cut-off value, endometrial cancer patients were divided into miR-143 high expression group and low expression group. The relationship between clinical characteristics and miR-143 expression was analyzed. After 3 years of follow-up, the survival rate was recorded, and the correlation between miR-143 expression and prognosis was analyzed.Results The expression level of miR-143 in the study group was lower than that in the control group (P<0.05), and the expression levels of miR-143 in cancer tissues and adjacent tissues of the dead in the study group were lower than those in the survival group (P<0.05). ROC curve analysis showed that the optimal cutoff value of cancer tissue in patients with endometrial cancer was 0.510, the area under the curve was 0.749, the sensitivity was 62.71%, the specificity was 80.21%, and the 95%CI was 1.320-3.440. There was statistically significant difference in the expression level of miR-143 among different pathological stages, lymph node metastasis and differentiation degree (P<0.05); while there was no significant difference in the expression level of miR-143 between different age, menopausal status, tumor diameter and muscle infiltration status (P>0.05). After 3 years of follow-up, the survival rate of high expression of miR-143 in cancer tissues of the study group was 60.00%, which was higher than 38.46% of low expression of miR-143, and the difference was statistically significant (P<0.05).Conclusion The expression level of miR-143 in endometrial cancer tissues is reduced, and its low expression has a significant impact on the prognosis of endometrial cancer patients. The expression of miR-143 can be used as an evaluation index for the prognosis of endometrial cancer in clinic.
