目的 研究KAI1和Snail、Slug、E-cadherin在鼻咽癌中的表达情况,分析其与患者预后的关系。方法 收集2011年6月-2015年7月赣南医学院第一附属医院病理科存档石蜡包埋的212例鼻咽癌组织标本,50例鼻咽部黏膜炎组织标本,正常鼻咽上皮组织标本40例,采用免疫组化方法检测各标本中KAI1和Snail、Slug、E-cadherin的表达,通过Kaplan-Meier法评价KAI1和Snail、Slug、E-cadherin的表达与鼻咽癌患者预后的关系,采用Cox回归分析患者临床特征对其生存的影响。结果 KAI1和Snail、Slug、E-cadherin在鼻咽癌标本中的表达阳性率分别为25.94%、65.09%、65.09%和31.60%;鼻咽癌组织中Snail、Slug表达阳性率高于鼻咽部黏膜炎组织和正常鼻咽上皮组织(P<0.05);鼻咽癌组织中KAI1、E-cadherin的表达阳性率低于鼻咽部黏膜炎组织和正常鼻咽上皮组织(P<0.05);KAI1与Snail(r=-0.435)、Slug(r=-0.473)呈负相关,与E-cadherin呈正相关(r=0.325);Kaplan-Meier分析显示,Snail(+)和Slug(+)患者的总生存时间短于Snail(-)和Slug(-)患者,KAI1(+)和E-cadherin(+)患者的总生存期长于KAI1(-)和E-cadherin(-)患者(P<0.05);Cox回归分析发现,KAI1、Snail、Slug、E-cadherin、淋巴结转移、远处转移、TNM分期是影响鼻咽癌患者预后的独立因素。结论 KAI1和Snail、Slug、E-cadherin的表达及EB-DNA水平、肿瘤大小、淋巴结转移、远处转移、TNM分期与鼻咽癌预后相关,联合检测可辅助判断鼻咽癌患者的预后。
Abstract:
Objective To study the expression of KAI1, Snail, Slug, and E-cadherin in nasopharyngeal carcinoma, and to analyze their relationship with the prognosis of patients.Methods From June 2011 to July 2015, 212 paraffin-embedded specimens of nasopharyngeal carcinoma, 50 specimens of chronic nasopharynx mucositis and 40 specimens of normal nasopharynx epithelium were collected from the Department of Pathology of the First Affiliated Hospital of Gannan Medical College. The expressions of KAI1, Snail, Slug and E-cadherin in each specimen were detected by immunohistochemistry. The relationship between the expressions of KAI1, Snail, Slug and E-cadherin and the prognosis of patients with nasopharyngeal carcinoma was evaluated by Kaplan-Meier method. Cox regression was used to analyze the influence of clinical characteristics of patients on their survival.Results The positive expression rates of KAI1, Snail, Slug and E-cadherin in nasopharyngeal carcinoma specimens were 25.94%, 65.09%, 65.09% and 31.60%, respectively. The positive rates of Snail and Slug expression in nasopharyngeal carcinoma specimens were higher than those in chronic pharyngitis specimens and normal nasopharyngeal mucosa specimens (P<0.05). The positive expression rates of KAI1 and E-cadherin in nasopharyngeal carcinoma specimens were lower than those in chronic nasopharyngitis specimens and normal nasopharyngeal mucosa specimens (P<0.05). KAI1 was negatively correlated with Snail (r=-0.435) and Slug (r=-0.473), and positively correlated with E-cadherin (r=0.325). Kaplan-Meier analysis showed that the overall survival time of Snail(+) and Slug(+) patients was shorter than that of Snail(-) and Slug(-) patients, and the overall survival time of KAI1(+) and E-cadherin(+) patients was longer than that of KAI1(-) and E-cadherin(-) patients (P<0.05). Cox regression analysis showed that KAI1, Snail, Slug, E-cadherin, lymph node metastasis, distant metastasis, and TNM stage were independent factors affecting the prognosis of patients with nasopharyngeal carcinoma.Conclusion The expression of KAI1 and Snail, Slug, E-cadherin is associated with EB-DNA level, tumor size, lymph node metastasis, distant metastasis, TNM stage and prognosis of nasopharyngeal carcinoma. Combined detection can help determine the prognosis of patients with nasopharyngeal carcinoma.
