摘要:
目的 研究输尿管镜下钬激光碎石治疗输尿管结石的临床效果及对症状转归的影响。方法 选取2021年4月-2022年4月在我院诊治的56例输尿管结石患者为研究对象，采用随机数字表法分为对照组（n=28）和观察组（n=28），对照组采用体外冲击波碎石术治疗，观察组采用输尿管镜下钬激光碎石治疗，比较两组一次碎石成功率、结石清除率、结石排净时间、手术指标、疼痛评分及并发症发生率。结果 观察组一次碎石成功率为92.86%，高于对照组的78.57%，结石清除率为96.43%，高于对照组的82.14%（P＜0.05）；观察组结石排净时间、术后血尿时间、住院时间短于对照组，手术时间、术中出血量、胃肠功能恢复时间均长于对照组（P＜0.05）；观察组术后12、24 h疼痛评分均低于对照组（P＜0.05）；观察组并发症发生率为7.14%，低于对照组的21.43%（P＜0.05）。结论 输尿管镜下钬激光碎石治疗输尿管结石临床效果确切，结石清除率高，不仅可缩短结石排净时间，而且可减轻疼痛、缩短术后血尿时间，降低并发症发生率，是一种安全、可行的治疗手段，值得临床应用。
Abstract:
Objective To study the clinical effect of ureteroscopic holmium laser lithotripsy in the treatment of ureteral calculi and its effect on symptom outcome.Methods A total of 56 patients with ureteral calculi diagnosed and treated in our hospital from April 2021 to April 2022 were selected as the research objects. They were divided into control group (n=28) and observation group (n=28) by random number table method. The control group was treated with extracorporeal shock wave lithotripsy, and the observation group was treated with ureteroscopic holmium laser lithotripsy. The success rate of one-time lithotripsy, stone clearance rate, stone clearance time, surgical index, pain score and complication rate were compared between the two groups.Results The success rate of one-time lithotripsy in the observation group was 92.86%, which was higher than 78.57% in the control group, and the stone clearance rate was 96.43%, which was higher than 82.14% in the control group (P<0.05). The stone clearance time, postoperative hematuria time and hospitalization time in the observation group were shorter than those in the control group, and the operation time, intraoperative blood loss and gastrointestinal function recovery time were longer than those in the control group (P<0.05). The pain scores of the observation group at 12 and 24 h after operation were lower than those of the control group (P<0.05). The incidence of complications in the observation group was 7.14%, which was lower than 21.43% in the control group (P<0.05).Conclusion The clinical effect of ureteroscopic holmium laser lithotripsy in the treatment of ureteral calculi is definite, which can improve the stone clearance rate, shorten the stone clearance time and postoperative hematuria time, relieve pain and reduce the incidence of complications. It is a safe and feasible treatment method.
