摘要:
目的 研究尿蛋白和尿微量白蛋白在糖尿病肾病检验中的临床应用价值。方法 选取2020年2月-2022年2月在我院诊治的100例糖尿病肾病患者为观察组,并选取同期我院体检健康者100名为对照组,比较两组尿蛋白、尿微量白蛋白水平和检出阳性率、不同病程和不同分期糖尿病肾病患者尿蛋白、尿微量白蛋白水平,分析尿蛋白、尿微量白蛋白和两者联合诊断糖尿病肾病的敏感度、特异度。结果 观察组尿蛋白、尿微量白蛋白水平均高于对照组(P<0.05);观察组尿蛋白、尿微量白蛋白检出阳性率均高于对照组(P<0.05);糖尿病肾病病程>12年的患者尿蛋白、尿微量白蛋白水平均高于病程6~12年、<6年的患者,且病程6~12年的患者尿蛋白、尿微量白蛋白水平高于病程<6年的患者(P<0.05);糖尿病肾病Ⅴ期患者尿蛋白、尿微量白蛋白水平高于Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期患者,Ⅳ期患者尿蛋白、尿微量白蛋白水平高于Ⅰ期、Ⅱ期、Ⅲ期患者,Ⅲ期高于患者尿蛋白、尿微量白蛋白水平高于Ⅰ期、Ⅱ期患者(P<0.05);尿蛋白联合尿微量白蛋白诊断糖尿病肾病的敏感度、特异度均高于尿蛋白、尿微量白蛋白单独诊断糖尿病肾病,且尿微量白蛋白诊断糖尿病肾病的敏感度、特异度高于尿蛋白(P<0.05)。结论 糖尿病肾病患者尿蛋白和尿微量白蛋白水平和阳性率均升高,且随病程、肾病分期升高而升高,两者联合检测可提高糖尿病肾病的诊断敏感度、特异度,为早期治疗和预防改变提供参考依据。
Abstract:
Objective To study the clinical application value of urinary protein and urinary microalbumin in the examination of diabetic nephropathy.Methods A total of 100 patients with diabetic nephropathy who were diagnosed and treated in our hospital from February 2020 to February 2022 were selected as the observation group, and 100 healthy people who underwent physical examination in our hospital during the same period were selected as the control group. The levels of urinary protein, urinary microalbumin and the positive rate of detection were compared between the two groups. The levels of urinary protein and urinary microalbumin in patients with different course of disease and different stages of diabetic nephropathy were analyzed. The sensitivity and specificity of urinary protein, urinary microalbumin and their combination in the diagnosis of diabetic nephropathy were analyzed.Results The levels of urinary protein and urinary microalbumin in the observation group were higher than those in the control group (P<0.05). The positive rates of urinary protein and urinary microalbumin in the observation group were higher than those in the control group (P<0.05). The levels of urinary protein and urinary microalbumin in patients with diabetic nephropathy course >12 years were higher than those in patients with diabetic nephropathy course of 6-12 years and <6 years, and the levels of urinary protein and urinary microalbumin in patients with diabetic nephropathy course of 6-12 years were higher than those in patients with diabetic nephropathy course of <6 years (P<0.05). The levels of urinary protein and urinary microalbumin in patients with stage Ⅴdiabetic nephropathy were higher than those in patients with stage Ⅰ, stage Ⅱ, stage Ⅲ and stage Ⅳ, the levels of urinary protein and urinary microalbumin in patients with stage Ⅳ were higher than those in patients with stage Ⅰ, stage Ⅱ and stage Ⅲ, and the levels of urinary protein and urinary microalbumin in patients with stage Ⅲ were higher than those in patients with stage Ⅰ and stage Ⅱ (P<0.05). The sensitivity and specificity of urinary protein combined with urinary microalbumin in the diagnosis of diabetic nephropathy were higher than those of urinary protein and urinary microalbumin alone in the diagnosis of diabetic nephropathy, and the sensitivity and specificity of urinary microalbumin in the diagnosis of diabetic nephropathy were higher than those of urinary protein (P<0.05).Conclusion The levels and positive rates of urinary protein and urinary microalbumin in patients with diabetic nephropathy increase, and increase with the course of disease and the stage of nephropathy. The combined detection of the two can improve the diagnostic sensitivity and specificity of diabetic nephropathy, and provide reference for early treatment and prevention of changes.
