摘要:
目的 研究布托啡诺复合右美托咪定在剖宫产麻醉中的应用效果及对应激反应的影响。方法 选取2021年4月-2023年4月在我院行剖宫产的60例产妇为研究对象，采用随机数字表法分为对照组和观察组，各组30例。对照组采用布托啡诺单纯麻醉，观察组在对照组基础上复合右美托咪定麻醉，比较两组麻醉起效时间、术后苏醒时间、剖宫产术后疼痛（VAS）评分，镇静（Ramsay）评分、应激反应[去甲肾上腺素（NE）、内皮素-1（ET-1）]及不良反应发生情况。结果 观察组麻醉起效时间、术后苏醒时间均短于对照组（P＜0.05）；观察组术后6、12、24 h VAS评分均低于对照组（P＜0.05）；观察组术后6 h Ramsayp评分与对照组比较，差异无统计学意义（P＞0.05），观察组术后12、24 h Ramsayp评分均低于对照组（P＜0.05）；两组术后1 h NE、ET-1比较，差异无统计学意义（P＞0.05），两组术后12 h NE、ET-1均高于术后1 h，但观察组NE、ET-1低于对照组（P＜0.05）；观察组恶心、呕吐发生率低于对照组（P＜0.05），两组头晕、心动过缓、低血压发生率比较，差异无统计学意义（P＞0.05）。结论 布托啡诺复合右美托咪定在剖宫产麻醉中的应用效果良好，可减轻产妇术后疼痛和应激反应，且起效快，术后苏醒时间短，恶心、呕吐不良反应发生率低，更具有应用优势。
Abstract:
Objective To study the application effect of butorphanol combined with dexmedetomidine in cesarean section anesthesia and its effect on stress response.Methods Sixty parturients who underwent cesarean section in our hospital from April 2021 to April 2023 were selected as the research objects. They were divided into control group and observation group by random number table method, with 30 parturients in each group. The control group was anesthetized with butorphanol alone, and the observation group was anesthetized with dexmedetomidine on the basis of the control group. The onset time of anesthesia, postoperative recovery time, pain (VAS) score after cesarean section, sedation (Ramsay) score, stress response [norepinephrine (NE), endothelin-1 (ET-1)] and adverse reactions were compared between the two groups.Results The onset time of anesthesia and postoperative recovery time in the observation group were shorter than those in the control group (P<0.05). The VAS scores of the observation group at 6,12 and 24 h after operation were lower than those of the control group (P<0.05). There was no significant difference in Ramsayp score between the observation group and the control group at 6 h after operation (P>0.05). The Ramsayp scores of the observation group at 12 and 24 h after operation were lower than those of the control group (P<0.05). There was no significant difference in NE and ET-1 between the two groups at 1 h after operation (P>0.05),wihle the NE and ET-1 of the two groups at 12 h after operation were higher than those at 1 h after operation, the NE and ET-1 of the observation group were lower than those of the control group (P<0.05). The incidence of nausea and vomiting in the observation group was lower than that in the control group (P<0.05). There was no significant difference in the incidence of dizziness, bradycardia and hypotension between the two groups (P>0.05).Conclusion The application effect of butorphanol combined with dexmedetomidine in cesarean section anesthesia is good, which can reduce the postoperative pain and stress response of parturients, and has the advantages of rapid onset, short postoperative recovery time, low incidence of adverse reactions of nausea and vomiting, and more application advantages.
