摘要:
目的 调查北京市复兴医院老年科老年多病共存住院患者的临床特点及护理需求，分析其特点，为此类患者的临床护理工作提供理论依据。方法 选取2016年1月1日~12月31日在复兴医院老年科≥60岁的348例住院患者，统计其临床特点，包括绝对卧床、压疮例数；住院期间发生急性并发症例数；护理需求，包括预防压疮护理、压疮护理、鼻饲、深静脉置管、PICC置管、尿管、气管切开例数；用药途径及种类。结果 348例老年多病共存住院患者中，绝对卧床35.06%（122/348），压疮12.07%（42/348）；住院期间发生急性心力衰竭4.89%（17/348）、药物性肝损害2.30%（8/348）、急性胃肠功能紊乱7.47%（26/348）、急性消化道出血4.31%（15/348），急性下肢深静脉血栓形成5.46%（19/348），跌倒1.15%（4/348），皮肤瘀斑及皮下出血2.30%（8/348），吸入性肺炎7.76%（27/348），导管感染0.57%（2/348）。预防压疮护理35.06%（122/348），压疮护理12.07%（42/348），鼻饲24.71%（86/348），深静脉置管9.77%（34/348）、PICC置管27.59%（96/348），留置尿管6.90%（24/348），气管切开1.44%（5/348）。用药途径1~6种，用药种类2~16种。结论 老年多病共存住院患者存在多重用药，住院期间容易出现多种急性并发症，护理需求量大且护理难度高，需要对这部分患者采取综合护理以改善患者预后。
Abstract:
Abstract：Objective To investigate the clinical characteristics and nursing needs of elderly patients with multiple diseases in the elderly in Beijing Fuxing Hospital,and to analyze their characteristics,and to provide theoretical basis for clinical nursing work of such patients.Methods A total of 348 inpatients aged 60 years or older in Fuxing Hospital from January 1 to December 31,2016 were enrolled.The clinical characteristics,including the number of absolute bed rest and pressure ulcers,and the number of acute complications during hospitalization were collected;Requirements,including prevention of pressure sore care,pressure sore care,nasal feeding,deep vein catheterization,PICC catheterization,urinary catheter,tracheotomy,number of routes and types of medication.Results Of the 348 elderly patients with multiple disease coexistence,35.06%(122/348)of absolute bed rest,12.07%(42/348)of pressure ulcers; 4.89%(17/348)of acute heart failure,drug-induced liver damage 2.30%(8/348),acute gastrointestinal dysfunction 7.47%(26/348),acute gastrointestinal bleeding 4.31%(15/348),acute lower extremity deep vein thrombosis 5.46%(19/348),fell 1.15%(4/348),skin freckle and subcutaneous hemorrhage 2.30%(8/348),aspiration pneumonia 7.76%(27/348),catheter infection 0.57%(2/348).Prevention of pressure ulcer care 35.06%(122/348),pressure ulcer care 12.07%(42/348),nasal feeding 24.71%(86/348),deep vein catheterization 9.77%(34/348),PICC catheterization 27.59%(96/348),indwelling catheter 6.90%(24/348),tracheotomy 1.44%(5/348).There are 1 to 6 kinds of drug routes,and 2 to 16 kinds of drugs are used.Conclusion There are multiple medications for elderly patients with multiple diseases.It is prone to multiple acute complications during hospitalization.The need for nursing is high and the nursing is difficult. Comprehensive care is needed to improve the prognosis of these patients.
