摘要:
目的 研究布地奈德联合鼻内镜手术治疗鼻窦炎-鼻息肉的疗效及对患者鼻腔功能的影响。方法 选取2018年1月-2021年12月在我院诊治的120例鼻窦炎-鼻息肉患者为研究对象，采用随机数字表法分为对照组和观察组，各60例。两组均采用鼻内镜手术治疗，对照组术后给予生理盐水喷鼻，观察组术后给予布地奈德喷鼻，比较两组临床症状消退时间、鼻腔黏膜结构评分（Lund-Mackey和 Lund-Kenned）、嗅觉功能评分、鼻气道阻力、鼻纤毛清除率、并发症发生率及复发率。结果 观察组鼻塞、鼻漏、头痛消退时间均短于对照组（P＜0.05）；两组治疗后Lund-Mackey、Lund-Kenned评分均低于治疗前，且观察组低于对照组（P＜0.05）；两组治疗后嗅觉功能评分、鼻气道阻力均小于治疗前，鼻纤毛清除率大于治疗前，且观察组嗅觉功能评分、鼻气道阻力均小于对照组，鼻纤毛清除率大于对照组（P＜0.05）；观察组并发症发生率为6.67%，低于对照组的15.00%（P＜0.05）；随访3个月，观察组复发率为5.00%，低于对照组的11.67%（P＜0.05）。结论 布地奈德联合鼻内镜手术治疗鼻窦炎-鼻息肉疗效确切，可提高鼻纤毛清除率，减小鼻气道阻力，患者术后恢复快，鼻腔功能恢复良好，且并发症发生率和复发率低，具有较理想的应用有效性和安全性。
Abstract:
Objective To study the efficacy of budesonide combined with endoscopic sinus surgery in the treatment of sinusitis-nasal polyps and its effect on nasal function.Methods A total of 120 patients with sinusitis-nasal polyps diagnosed and treated in our hospital from January 2018 to December 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 60 cases in each group. Both groups were treated with nasal endoscopic surgery. The control group was given normal saline nasal spray after operation, and the observation group was given budesonide nasal spray after operation. The clinical symptom regression time, nasal mucosal structure score (Lund-Mackey and Lund-Kenned), olfactory function score, nasal airway resistance, nasal cilia clearance rate, complication rate and recurrence rate were compared between the two groups.Results The disappearance time of nasal congestion, rhinorrhea and headache in the observation group was shorter than that in the control group (P<0.05). After treatment, the scores of Lund-Mackey and Lund-Kennedy in the two groups were lower than those before treatment, and those in the observation group were lower than the control group (P<0.05). After treatment, the olfactory function score and nasal airway resistance of the two groups were lower than those before treatment, and the nasal cilia clearance rate was higher than that before treatment, manwhile the olfactory function score and nasal airway resistance of the observation group were lower than those of the control group, and the nasal cilia clearance rate was higher than that of the control group (P<0.05). The incidence of complications in the observation group was 6.67%, which was lower than 15.00% in the control group (P<0.05). After 3 months of follow-up, the recurrence rate of the observation group was 5.00%, which was lower than 11.67% of the control group (P<0.05).Conclusion Budesonide combined with nasal endoscopic surgery is effective in the treatment of sinusitis-nasal polyps, which can improve the clearance rate of nasal cilia and reduce the resistance of nasal airway. Meanwhile, the patients recover quickly after operation, the function of nasal cavity recovers well, and the incidence of complications and recurrence rate are low.
