摘要:
目的 探讨阿奇霉素联合布地奈德雾化治疗小儿支气管肺炎的效果及对患儿临床症状的影响。方法 选取2021年2月-2023年8月上高县妇幼保健院收治的60例支气管肺炎患儿作为研究对象,按照随机数字表法将其分为观察组和对照组,各30例。对照组给予阿奇霉素治疗,观察组给予阿奇霉素联合布地奈德雾化治疗,对两组临床疗效、临床症状消失时间(气喘消失时间、退热时间、咳嗽消失时间、肺啰音消失时间)、炎性因子水平[白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]、肺功能[用力肺活量(FVC)、一秒用力呼气容积(FEV1)、最大呼气流量(PEF)、最大呼吸中段流速(MEF25-75)、用力呼气50%流速(MEF50)及用力呼气25%流速(MEF25)]进行比较。结果 观察组治疗总有效率(96.67%)高于对照组(73.33%)(P<0.05);相较于对照组,观察组气喘消失时间、退热时间、咳嗽消失时间、肺啰音消失时间均更短(P<0.05);两组治疗后炎性因子(IL-8、IL-6、TNF-α、CRP)水平均低于治疗前,且相较于对照组,观察组治疗后炎性因子(IL-8、IL-6、TNF-α、CRP)水平均更低(P<0.05);两组治疗后肺功能指标(FVC、FEV1、PEF、MEF25、MEF50、MEF25-75)水平均高于治疗前,且相较于对照组,观察组治疗后肺功能指标(FVC、FEV1、PEF、MEF25、MEF50、MEF25-75)水平均更高(P<0.05)。结论 阿奇霉素联合布地奈德雾化治疗支气管肺炎的效果确切,能够缩短患儿临床症状消失时间,减轻炎性反应,提高肺功能,值得临床使用。
Abstract:
Objective To investigate the effect of azithromycin combined with budesonide atomization in the treatment of bronchopneumonia in children and its effect on clinical symptoms. Methods From February 2021 to August 2023, 60 pediatric patients with bronchopneumonia admitted to Shanggao County Maternal and Child Health Hospital were enrolled and randomly allocated into an observation group (n=30) and a control group (n=30). The control group received azithromycin monotherapy, while the observation group received azithromycin along with budesonide. The clinical efficacy, the disappearance time of clinical symptoms (asthma disappearance time, antipyretic time, cough disappearance time and lung rale disappearance time), inflammatory marker levels [interleukin-6 (IL-6), interleukin-8 (IL-8), tumor necrosis factor-alpha (TNF-α) and C-reactive protein (CRP)] and pulmonary function parameters [forced vital capacity (FVC), forced expiratory volume in one second (FEV1), maximum expiratory flow (PEF), maximum mid-expiratory flow rate (MEF25-75), and forced expiratory flow rates at 50% (MEF50) and 25% (MEF25)] were compared between the groups. Results The total effective rate of treatment in the observation group (96.67%) was higher than that in the control group (73.33%) (P<0.05). Compared with the control group, the disappearance time of clinical symptoms such as asthma disappearance time, antipyretic time, cough disappearance time and lung rale disappearance time in the observation group were shorter (P<0.05). The levels of inflammatory factors (IL-8, IL-6, TNF-α, CRP) in the two groups after treatment were lower than those before treatment, and compared with the control group, the levels of inflammatory factors (IL-8, IL-6, TNF-α, CRP) in the observation group after treatment were lower(P<0.05). The levels of lung function indexes (FVC, FEV1, PEF, MEF25, MEF50, MEF25-75) in the two groups after treatment were higher than those before treatment, and compared with the control group, the levels of lung function indexes (FVC, FEV1, PEF, MEF25, MEF50, MEF25-75) in the observation group after treatment were higher (P<0.05). Conclusion Azithromycin combined with budesonide atomization is effective in the treatment of bronchopneumonia, which can shorten the disappearance time of clinical symptoms, reduce inflammatory reaction and improve lung function. It is worthy of clinical use.
