摘要:
目的 分析氯吡格雷与阿司匹林联合治疗对急性心肌梗死患者心功能、凝血功能的影响。方法 2021年7月-2024年6月，从安福县人民医院选取60例急性心肌梗死患者，采用随机数字表法进行分组：对照组30例，研究组30例。两组均予以常规对症治疗，对照组在其基础上予以阿司匹林肠溶片治疗，研究组在对照组基础上加用氯吡格雷治疗。比较两组临床疗效、心功能指标[左室射血分数（LVEF）、左室舒张末期内径（LVEDD）、左室收缩末期内径（LVESD）]、凝血功能[活化部分凝血活酶时间（APTT）、纤维蛋白原（Fg）、血浆凝血酶时间（TT）、血浆凝血酶原时间（PT）]、心血管不良事件发生率（心律失常、血管再闭塞、心力衰竭、心血管性死亡）。结果 研究组治疗总有效率（96.67%）高于对照组（73.33%）（P＜0.05）；治疗后，研究组在心功能指标（LVEF更高，LVEDD、LVESD更低）和凝血功能指标（APTT、TT、PT更高，Fg更低）方面均较对照组更优（P＜0.05）；研究组心血管不良事件发生率（3.33%）低于对照组（23.33%）（P＜0.05）。结论 对于急性心肌梗死患者，氯吡格雷与阿司匹林联合治疗可提升临床疗效，提高患者心功能，改善其凝血功能，且安全性较高。
Abstract:
Objective To analyze the effect of clopidogrel combined with aspirin on cardiac function and coagulation function in patients with acute myocardial infarction. Methods A total of 60 acute myocardial infarction patients admitted to Anfu County People′s Hospital between July 2021 and June 2024 were enrolled and randomly assigned to either a control group or a study group (30 patients per group) using a random number table method. Both groups were given routine symptomatic treatment. The control group was treated with aspirin enteric-coated tablets on the basis of the routine symptomatic treatment, and the study group was treated with clopidogrel on the basis of the control group. The clinical efficacy, cardiac function indexes [left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD)], coagulation function [activated partial thromboplastin time (APTT), fibrinogen (Fg), plasma thrombin time (TT), plasma prothrombin time (PT)] and incidence of cardiovascular adverse events (arrhythmia, vascular reocclusion, heart failure, cardiovascular death) were compared between the two groups. Results The study group exhibited a higher total treatment effective rate (96.67%) compared to the control group (73.33%) (P<0.05). After treatment, the study group showed superior outcomes to the control group in terms of both cardiac function indexes(increased LVEF, decreased LVEDD and LVESD) and coagulation function indexes (prolonged APTT, TT, PT, and reduced Fg) (P<0.05). The incidence of cardiovascular adverse events in the study group (3.33%) was lower than that in the control group (23.33%) (P<0.05). Conclusion For patients with acute myocardial infarction, clopidogrel combined with aspirin can improve the clinical efficacy, improve their cardiac function and coagulation function, with higher safety.
