摘要:
目的 研究有创机械通气治疗慢性阻塞性肺疾病患者呼吸衰竭的效果。方法 选择2018年5月~2019年5月我院收治的慢性阻塞性肺疾病(COPD)呼吸衰竭患者110例,采用随机数字表法分为观察组和对照组,各55例。观察组采用有创机械通气治疗,对照组采用无创机械通气治疗。比较两组通气时间、住院时间、感染发生率、气管切开率以及血气指标和感染发生率。结果 观察组通气时间、住院时间均高于对照组,差异有统计学意义(P<0.05);气管切开率高于对照组,但差异无统计学意义(P>0.05);治疗后两组氧分压均高于治疗前,二氧化碳分压、心率、呼吸频率均低于治疗前,差异有统计学意义(P<0.05),但观察组与对照组比较,差异无统计学意义(P>0.05);观察组感染发生率高于对照组,差异有统计学意义(P<0.05 )。结论 有创机械通气治疗慢性阻塞性肺疾病患者呼吸衰竭效果确切,与无创机械通气治疗效果基本一致,但有创机械通气临床感染发生率较高,建议在临床机械通气治疗中优先选择无创机械通气治疗。
Abstract:
Objective To study the effect of invasive mechanical ventilation on respiratory failure in patients with chronic obstructive pulmonary disease. Methods A total of 110 patients with chronic obstructive pulmonary disease and respiratory failure treated in our hospital from May 2018 to May 2019 were selected and divided into observation group and control group by random number table method, with 55 cases in each group. The observation group was treated with invasive mechanical ventilation, and the control group was treated with noninvasive mechanical ventilation. The ventilation time, length of hospital stay, incidence of infection, tracheotomy rate, blood gas index and incidence of infection were compared between the two groups. Results The ventilation time and length of stay in the observation group were higher than those in the control group,the differences were statistically significant (P<0.05). The tracheotomy rate was higher than that in the control group, but the differences were not statistically significant (P> 0.05). The partial pressure was higher than before treatment, and the carbon dioxide partial pressure, heart rate, and respiratory rate were lower than before treatment,the differences were statistically significant (P<0.05), but there was no significant difference between the observation group and the control group (P>0.05). The incidence of infection in the observation group was higher than that in the control group,the difference was statistically significant (P<0.05).Conclusion The effect of invasive mechanical ventilation in patients with chronic obstructive pulmonary disease on respiratory failure is definite, and it is basically the same as the effect of noninvasive mechanical ventilation. However, the incidence of clinical infection in invasive mechanical ventilation is high. It is recommended to choose noninvasive mechanical ventilation in clinical mechanical ventilation treatment.
