摘要:
目的 研究彩色多普勒超声评估血液透析患者自体动静脉内瘘血流量及狭窄的价值。方法 回顾性分析2019年1月-2022年12月我院行血液透析的66例患者临床资料,均进行彩色多普勒超声检查,比较不同内瘘流量(内瘘流量正常和不足)、狭窄情况(正常和狭窄)患者肱动脉近分叉处、瘘口近端桡动脉、瘘口处、瘘口近端头静脉血流量;采用ROC检验超声检测以上指标诊断实际透析量不足和内瘘狭窄的效能(敏感度、特异度)。结果 内瘘流量正常者肱动脉近分叉处、瘘口近端桡动脉、瘘口处、瘘口近端头静脉血流量均大于内瘘流量不足者(P<0.05);内瘘狭窄者肱动脉近分叉处、瘘口近端桡动脉、瘘口处、瘘口近端头静脉血流量均小于内瘘正常者(P<0.05);ROC曲线分析显示,超声检测肱动脉近分叉处血流量诊断内瘘流量不足和内瘘狭窄的敏感度、特异度均高于瘘口近端桡动脉、瘘口处、瘘口近端头静脉血流量(P<0.05)。结论 彩色多普勒超声评估血液透析患者自体动静脉内瘘血流量及狭窄具有较高的价值,尤其是肱动脉近分叉处血流量评估的诊断效能较高,可为后续治疗提供可靠参考。
Abstract:
Objective To study the value of color Doppler ultrasound in evaluating the blood flow and stenosis of autogenous arteriovenous fistula in hemodialysis patients.Methods The clinical data of 66 patients who underwent hemodialysis in our hospital from January 2019 to December 2022 were retrospectively analyzed. Color Doppler ultrasonography was performed to compare the blood flow of brachial artery near bifurcation, proximal radial artery of fistula, fistula and proximal cephalic vein of fistula in patients with different internal fistula flow (normal and insufficient internal fistula flow) and stenosis (normal and stenosis). ROC test was used to test the efficacy (sensitivity, specificity) of the above indicators in the diagnosis of insufficient dialysis volume and fistula stenosis.Results The blood flow of the brachial artery near the bifurcation, the proximal radial artery of the fistula, the fistula, and the proximal end of the fistula in the patients with normal internal fistula flow were greater than those in the patients with insufficient internal fistula flow (P<0.05). The blood flow of the brachial artery near the bifurcation, the proximal radial artery of the fistula, the fistula, and the proximal end of the fistula in the patients with stenosis of the internal fistula were lower than those in the patients with normal internal fistula (P<0.05). ROC curve analysis showed that the sensitivity and specificity of ultrasound detection of blood flow at the brachial artery near bifurcation in the diagnosis of insufficient internal fistula flow and internal fistula stenosis were higher than those of the proximal radial artery of the fistula, the fistula, and the proximal venous blood flow of the fistula (P<0.05).Conclusion Color Doppler ultrasound has a high value in evaluating the blood flow and stenosis of autogenous arteriovenous fistula in hemodialysis patients, especially the diagnostic efficiency of blood flow assessment at the brachial artery near bifurcation, which can provide a reliable reference for subsequent treatment.
