摘要:
目的 分析氯吡格雷联合阿司匹林治疗老年冠心病心绞痛急性发作患者的临床疗效及对心功能的影响。方法 选择2021年1月-2023年11月德兴市中医院收治的老年冠心病心绞痛急性发作患者60例为研究对象，按照随机数字表法将其分为对照组和研究组，每组30例。对照组给予单一性阿司匹林肠溶片治疗，研究组给予氯吡格雷联合阿司匹林治疗。比较两组临床疗效、心功能[左心室舒张末期内径（LVEDD）、左心室射血分数（LVEF）、左心室收缩末期内径（LVESD）]、血液流变学水平[纤维蛋白原（FIB）、D二聚体（D-D）、血小板聚集率（PAR）]、炎性因子水平[白细胞介素（IL）-6、C反应蛋白（CRP）、肿瘤坏死因子（TNF）-α]、心绞痛发作情况（心绞痛发作次数、心绞痛持续时间、心绞痛发作间隔时间）。结果 研究组总有效率（96.67%）高于对照组（73.33%）（P＜0.05）；与治疗前比较，两组治疗后LVEDD、LVESD水平降低，LVEF水平升高，且与对照组比较，研究组治疗后LVEDD、LVESD水平更低，而LVEF水平更高（P＜0.05）；与治疗前比较，两组治疗后FIB、D-D、FAR水平降低，且与对照组比较，研究组治疗后FIB、D-D、FAR水平更低（P＜0.05）；与治疗前比较，两组治疗后IL-6、CRP、TNF-α水平降低，且与对照组比较，研究组治疗后IL-6、CRP、TNF-α水平更低（P＜0.05）；与对照组比较，研究组心绞痛发作次数更少、心绞痛持续时间更短、心绞痛发作间隔时间更长（P＜0.05）。结论 与单一性阿司匹林肠溶片治疗，氯吡格雷联合阿司匹林治疗老年冠心病心绞痛急性发作患者的临床疗效确切，可有效改善患者心功能和心绞痛发作情况，降低患者血液流变学和炎性因子水平。
Abstract:
Objective To analyze the clinical efficacy of clopidogrel combined with aspirin in the treatment of elderly patients with acute attack of coronary heart disease and its effect on cardiac function. Methods A total of 60 elderly patients with acute attack of angina pectoris of coronary heart disease admitted to Dexing Hospital of Traditional Chinese Medicine from January 2021 to November 2023 were selected as the research objects. According to the random number table method, they were divided into control group and study group, with 30 patients in each group. The control group was treated with single aspirin enteric-coated tablets, and the study group was treated with clopidogrel combined with aspirin. The clinical efficacy, cardiac function [left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), left ventricular end-systolic diameter (LVESD)], hemorheology levels [fibrinogen (FIB), D-dimer (D-D), platelet aggregation rate (PAR)], inflammatory factor levels [interleukin (IL) -6, C-reactive protein (CRP), tumor necrosis factor (TNF)-α], angina pectoris attack (angina pectoris attack frequency, angina pectoris duration, angina pectoris attack interval) were compared between the two groups. Results The total effective rate of the study group (96.67%) was higher than that of the control group (73.33%) (P<0.05). Compared with before treatment, the levels of LVEDD and LVESD in the two groups after treatment decreased, and the level of LVEF increased, while compared with the control group, the levels of LVEDD and LVESD in the study group were lower, the level of LVEF was higher after treatment (P<0.05). Compared with before treatment, the levels of FIB, D-D and FAR in the two groups after treatment decreased, and compared with the control group, the levels of FIB, D-D and FAR in the study group were lower after treatment (P<0.05). Compared with before treatment, the levels of IL-6, CRP and TNF-α in the two groups after treatment decreased, and compared with the control group, the levels of IL-6, CRP and TNF-α in the study group were lower after treatment (P<0.05). Compared with the control group, the number of angina pectoris attacks in the study group was less, the duration of angina pectoris was shorter, and the interval of angina pectoris attacks was longer (P<0.05). Conclusion Compared with single aspirin enteric-coated tablets, clopidogrel combined with aspirin has a definite clinical effect in the treatment of elderly patients with acute attack of coronary heart disease and angina pectoris, which can effectively improve the cardiac function and angina pectoris attack of patients, and reduce the levels of hemorheology and inflammatory factors.
