摘要:
目的 探讨组合式渐进呼吸康复训练结合心理干预在慢性阻塞性肺疾病患者中的应用效果,分析其对患者呼吸功能、肺功能、健康行为及心理状态的影响。方法 按照随机数字表法将2023年5月-2025年9月我院收治的66例慢阻肺患者分为对照组33例和观察组33例。对照组患者予以常规护理干预,观察组在对照组基础上予以组合式渐进呼吸康复训练+心理干预,两组均干预8周。比较两组护理前后呼吸功能指标[吸气峰压(PIP)、平均气道压(Pmean)、气道阻力(Raw)]、肺功能指标[最大中期呼气流速(FEF25%~75%)、50%肺活量时呼气流速(FEF50%)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC比值]、健康行为评分(饮食、运动、服药、戒烟酒)及心理状态评分(躯体化、焦虑、抑郁)。结果 护理8周后,与对照组相比,观察组PIP、Pmean、Raw更低(P<0.05);护理8周后,与对照组相比,观察组FVC、FEV1、FEV1/FVC、FEF25%~75%、FEF50%更高(P<0.05);与对照组相比,观察组健康行为各维度评分更高(P<0.05);与对照组相比,观察组躯体化、焦虑、抑郁评分均更低(P<0.05)。结论 组合式渐进呼吸康复训练结合心理干预能有效改善慢阻肺患者的呼吸功能和肺功能,促进健康行为形成,缓解负面心理状态,整体提升患者的康复效果和生活质量,值得在临床推广应用。
Abstract:
Objective To investigate the effect of combined progressive respiratory rehabilitation training and psychological intervention in patients with chronic obstructive pulmonary disease, and to analyze its impact on patients′ respiratory function, pulmonary function, health behavior, and mental status. Methods A total of 66 patients with chronic obstructive pulmonary disease admitted to the First People′s Hospital of Linchuan District, Fuzhou City from May 2023 to September 2025 were selected as the research objects. According to the random number table method, they were divided into control group (33 patients) and observation group (33 patients). In the control group, patients were given routine nursing intervention, and those in the observation group were given combined progressive respiratory rehabilitation training combined with psychological intervention on the basis of routine nursing. Both groups were intervened for 8 weeks. The respiratory function indexes [peak inspiratory pressure (PIP), mean airway pressure (Pmean), airway resistance (Raw)], pulmonary function indexes [maximal midexpiratory flow rate (FEF25%-75%), forced expiratory flow at 50% of FVC exhaled (FEF50%), forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), FEV1/FVC ratio], health behavior scores (diet, exercise, medication, quit smoking and alcohol) and mental status scores (somatization, anxiety, depression) were compared between the two groups before and after intervention. Results After 8 weeks of nursing, compared with the control group, the PIP, Pmean and Raw of the observation group were lower (P<0.05). After 8 weeks of nursing, compared with the control group, FVC, FEV1, FEV1/FVC, FEF25%-75% and FEF50% in the observation group were higher (P<0.05). Compared with the control group, the scores of each dimension of health behavior in the observation group were higher (P<0.05). Compared with the control group, the scores of somatization, anxiety and depression in the observation group were lower (P<0.05). Conclusion Combined progressive respiratory rehabilitation training combined with psychological intervention can effectively improve the respiratory function and pulmonary function of patients with chronic obstructive pulmonary disease, promote the formation of healthy behavior, alleviate the negative mental state, and improve the rehabilitation effect and quality of life of patients as a whole. It is worthy of clinical application.
